외상스트레스 장애(PTSD)[a]는 성폭행, 전쟁, 교통 충돌, 아동학대, 가정폭력, 그 밖의 사람의 삶에 대한 위협과 같은 외상적 사건에 노출되어 발병할 수 있는 정신적, 행동적 장애[6].[1][7]증상은 사건과 관련된 사고, 감정 또는 꿈, 외상 관련 단서에 대한 정신적 또는 육체적 고통, 외상 관련 단서들을 피하려는 시도, 사람이 생각하고 느끼는 방식의 변화, 전투-비행 대응의 증가를 포함할 수 있다.[1][3]이런 증상은 행사 후 한 달 이상 지속된다.[1]어린 아이들은 고통을 덜 느끼지만 놀이를 통해 기억을 표현할 수 있다.[1]PTSD를 앓고 있는 사람은 자살과 고의적인 자해의 위험이 더 높다.[2][8]

외상성 사건을 경험하는 대부분의 사람들은 PTSD를 발달시키지 않는다.[2]강간, 다른 성폭행, 납치, 스토킹, 친밀한 파트너에 의한 신체적 학대, 근친상간 또는 기타 형태의 아동 성폭력을 경험하는 사람들은 사고나 자연재해와 같은 비폭력에 기반한 트라우마를 경험하는 사람들보다 PTSD가 발병할 가능성이 더 높다.[9][10][11]노예제도, 강제수용소, 만성적인 가정학대 등 장기간에 걸친 트라우마를 겪는 사람들은 복합 외상후스트레스장애(C-PTSD)가 발생할 수 있다. C-PTSD는 PTSD와 유사하지만 사람의 정서조절과 핵심 정체성에 뚜렷한 영향을 미친다.[12]

상담이 초기 증상을 가진 사람들을 대상으로 할 때 예방이 가능할 수 있지만, 증상이 있는지 여부에 상관없이 모든 외상 노출 개인에게 제공될 때 효과적이지 않다.[2]PTSD를 앓고 있는 사람들을 위한 주요 치료법은 상담(심리치료)과 약물치료다.[3][13]SSRI 또는 SNRI 유형의 항우울제는 PTSD에 사용되는 첫 번째 약품이며, 약 절반의 사람들에게 적당하게 유익하다.[4]약물치료를 통해 얻는 이득은 상담과 함께 보이는 이익보다 적다.[2]약물 사용과 상담을 함께 하는 것이 두 방법 중 어느 한 방법보다 더 큰 이익이 있는지는 알려지지 않았다.[2][14]일부 SSRI 또는 SNRI를 제외한 의약품은 사용을 뒷받침할 충분한 증거가 없으며 벤조디아제핀의 경우 결과를 악화시킬 수 있다.[15][16]

미국에서는 성인의 약 3.5%가 특정 해에 PTSD를 가지고 있으며, 9%의 사람들이 인생의 어느 시점에 PTSD를 발병한다.[1]나머지 세계의 많은 지역에서, 주어진 한 해 동안의 비율은 0.5%에서 1% [1]사이이다.무력 충돌 지역에서 더 높은 비율이 발생할 수 있다.[2]그것은 남성보다 여성에게 더 흔하다.[3]

외상 관련 정신 질환의 증상은 적어도 고대 그리스 시대부터 기록되어 왔다.[17]1666년 런던 화재 이후 거슬리고 고통스러운 증상을 [18]묘사한 사무엘 페피스의 일기처럼 외상 후 질병의 증거가 17세기와 18세기부터 존재한다고 주장되어 왔다.세계 대전 중에는 '조개 쇼크' '전투 노이로제' 등 다양한 용어로 병세가 알려졌다.[19]'외상 후 스트레스 장애'라는 용어는 1970년대 베트남전 미군 참전용사들의 진단으로 크게 쓰였다.[20]1980년 미국 정신 질환 진단통계 설명서(DSM-III) 제3호에서 미국 정신 질환 협회에 의해 공식적으로 인정받았다.[21]

증상

서비스 회원들은 PTSD 증상을 완화하기 위해 예술을 사용한다.

PTSD의 증상은 일반적으로 외상성 사건을 일으킨 후 처음 3개월 이내에 시작되지만, 몇 년이 지나야 시작될 수 있다.[1][3]일반적인 경우, PTSD를 가진 개인은 외상 관련 생각과 감정 또는 외상 사건에 대한 토론을 끈질기게 피하고 심지어 그 사건에 대한 기억상실증을 가질 수도 있다.[1]그러나, 이 사건은 개인에 의해 거슬리고 반복적인 회상, 트라우마를 다시 경험하는 ("플래시백"), 악몽 (50~70%)[22][23]을 통해 일반적으로 재기된다.외상 후 증상이 나타나는 것은 흔한 일이지만 외상 후 1개월 이상 충분히 지속되어야 한다(즉, 외상 후 1개월 미만 동안 급성 스트레스일 수 있음).무질서하게 [1][24][25][26]하다어떤 사람들은 외상 후 성장을 경험한다.[27]

관련 의료 상태

외상 생존자들은 종종 PTSD 외에도 우울증, 불안장애, 그리고 기분장애가 발생한다.[28]

알코올 사용 장애와 같은 물질 사용 장애는 일반적으로 PTSD와 함께 발생한다.[29]외상 후 스트레스 장애 또는 기타 불안 장애로부터의 회복은 PTSD와 함께 약물 사용 장애를 동반할 때 방해되거나 상태가 악화될 수 있다.이러한 문제들을 해결하면 개인의 정신 건강 상태와 불안 수준이 향상될 수 있다.[30][31]

아동·청소년기의 경우 연령, 성별, 트라우마 유형과 무관하게 정서적 조절의 어려움(예: 기분 변화, 분노 폭발, 짜증)과 외상 후 스트레스 증상 사이에 강한 연관성이 있다.[32]

위험요소

위험 인물로는 전투병력, 자연재해 피해자, 강제수용소 생존자, 강력범죄 피해자 등이 있다.폭력(군인 등)이나 재난(응급근무자 등)에 노출되는 직업에 종사하는 사람도 위험에 처한다.[34]위험이 더 높은 직업에는 은행, 우체국 또는 상점에서 일하는 사람들 외에 경찰관, 소방관, 구급차 직원, 건강관리 전문가, 훈련 운전자, 다이버, 기자, 선원이 포함된다.[35]

외상

PTSD는 광범위한 외상 사건과 연관되어 있다.외상 후 PTSD 발병 위험은 외상 유형에[36][37] 따라 다르며 성폭력(11.4%), 특히 강간(19.0%)[38] 다음으로 높다.남성은 (어떤 유형이든) 외상성 사건을 경험할 가능성이 높지만 여성은 대인관계 폭력, 성폭행 등 PTSD로 이어질 수 있는 일종의 높은 충격성 외상성 사건을 경험할 가능성이 높다.[39]

어린이와 성인 모두 자동차 충돌 생존자들은 PTSD의 위험성을 증가시키고 있다.[40][41]전세계적으로 성인 중 약 2.6%가 생명을 위협하는 교통사고 이후 PTSD 진단을 받고 있으며, 비슷한 비율의 어린이들이 PTSD를 앓고 있다.[38]생명을 위협하는 자동차 사고에서 PTSD의 위험은 거의 두 배인 4.6%이다.[38]교통사고가 어린 시절 발생했든, 성인이 되었든 간에, 여성들은 도로 교통사고 이후 PTSD 진단을 받을 가능성이 더 높았다.[40][41]

외상 후 스트레스 반응은 어린이와 청소년들에게 연구되어 왔다.[42]PTSD의 비율은 성인보다 어린이에게서 낮을 수 있지만, 치료가 없을 때는 증상이 수십 년 동안 지속될 수도 있다.[43]한 추정에 따르면 선진국의 비와탄 인구에서 소아청소년 PTSD가 있는 비율은 성인의 1.5~3%에 비해 1%일 수 있다.[43]평균적으로 외상성 사건에 노출된 어린이의 16%가 PTSD를 발병하는데, 이는 노출 유형과 성별에 따라 달라진다.[44]성인 인구와 유사하게, 어린이의 PTSD에 대한 위험 요소에는 여성 성별, 재해에 대한 노출(자연적 또는 인공적), 부정적인 대처 행동 및/또는 적절한 사회적 지원 시스템 부족이 포함된다.[45]

예측 변수 모델들은 유년기의 외상, 만성 역경, 신경생물학적 차이, 가족적인 스트레스 요인들이 성인기의 외상 사건 이후 PTSD의 위험과 연관되어 있다는 것을 일관되게 밝혀 왔다.[46][47][48]예측하는 사건들의 일관된 측면을 찾기는 어려웠지만, 심층적인 이질화는 PTSD의 발전에 대한 상당히 일관된 예측 지표였다.[49]외상과의 근접성, 지속시간 및 중증도가 영향을 미친다.대인관계 트라우마가 비인격적 트라우마보다 더 많은 문제를 일으킨다는 추측이 나왔지만 이는 논란이 되고 있다.[50][51]신체적 학대, 폭행 또는 유괴에 노출된 개인에게 PTSD 발병 위험이 증가한다.[52][53]신체적 폭력을 경험한 여성은 남성보다 PTSD가 발달할 가능성이 높다.[52]

친밀한 파트너 폭력

가정폭력에 노출된 개인은 PTSD가 발달하는 경향이 있다.그러나, 충격적인 경험에 노출된다고 해서 개인이 PTSD를 개발할 것이라는 것을 자동적으로 나타내는 것은 아니다.[54]임신중기 가정폭력을 경험한 산모들의 PTSD 발달 사이에는 강한 연관성이 있다.[55]

성폭행이나 성폭행을 경험한 사람은 PTSD의 증상이 나타날 수 있다.[56][57] PTSD 증상은 폭행, 무감각, 불안감 증가와 관련된 것들을 피하고 놀라운 반응을 증가시키는 것을 포함한다.강간범이 그 사람을 감금하거나 제지할 경우, 강간당한 사람이 그들을 죽일 것이라고 믿었을 경우, 강간당한 사람은 매우 어렸거나 나이가 많았을 때, 그리고 만약 그 강간범이 그들이 알고 있는 사람이었다면, PTSD의 지속적인 증상의 가능성은 더 높다.생존자 주변 사람들이 강간 생존자를 무시하거나 비난할 경우(또는 무지한 경우) 심각한 증상이 나타날 가능성도 더 높다.[58]

전쟁관련외상

군 복무는 PTSD 개발의 위험 요소다.[59]전투에 노출된 사람들의 약 78%는 PTSD를 개발하지 않고, PTSD를 개발하는 군인의 약 25%에서는, 그 외모가 지연되고 있다.[59]

난민들은 또한 전쟁, 고난, 그리고 충격적인 사건에 노출되어 PTSD에 대한 위험도 증가하고 있다.난민 인구 내 PTSD 비율은 4%에서 86%[60]까지 다양하다.전쟁의 스트레스가 관련자 모두에게 영향을 미치는 반면, 실향민들은 다른 사람들보다 더 많은 것으로 나타났다.[61]

난민들의 전반적인 정신사회적 행복과 관련된 도전은 복잡하고 개별적으로 뉘앙스가 있다.난민들은 과거와 현재 진행 중인 트라우마로 인해 웰빙의 수준이 떨어지고 정신적 고통의 비율이 높다.특히 영향을 받고 필요를 충족하지 못하는 경우가 많은 그룹은 여성, 노인 및 동반하지 않은 미성년자들이다.[62]외상 후 스트레스와 난민 인구의 우울증도 그들의 교육적 성공에 영향을 미치는 경향이 있다.[62]

사랑하는 사람의 예상치 못한 죽음

사랑하는 사람의 갑작스러운 예기치 못한 죽음은 국가간 연구에서 보고된 가장 흔한 외상성 사건 유형이다.[38][63]그러나 이러한 유형의 행사를 경험하는 대다수의 사람들은 PTSD를 개발하지 않을 것이다.세계보건기구(WHO) 세계정신건강조사(WHO)의 분석 결과 사랑하는 사람의 예상치 못한 죽음을 알게 된 후 PTSD 발병 위험이 5.2%로 나타났다.[63]이런 종류의 외상성 사건의 유행이 높기 때문에, 사랑하는 사람의 예상치 못한 죽음은 전세계 PTSD 사례의 약 20%를 차지한다.[38]

생명을 위협하는 병

PTSD의 위험 증가와 관련된 의학적인 조건에는 암,[64][65][66] 심장마비,[67] 뇌졸중이 포함된다.[68]평생 PTSD를 앓고 있는 암 생존자의 22%가 증상을 좋아한다.[69]중환자실(ICU) 입원도 PTSD의 위험 요인이다.[70]어떤 여성들은 유방암유방절제술과 관련된 경험으로부터 PTSD를 경험한다.[71][72][64]생명을 위협하는 질병을 경험하는 사람들 중 사랑하는 사람들은 만성 질환을 가진 아이의 부모와 같이 PTSD에 걸릴 위험이 있다.[73]

임신 관련 외상

유산을 경험한 여성들은 PTSD의 위험에 처해있다.[74][75][76]이후 유산을 경험하는 사람들은 단 한 번의 유산을 경험하는 사람들에 비해 PTSD의 위험성이 더 높다.[74]PTSD는 출산 후에도 발생할 수 있으며, 여성이 임신 전에 트라우마를 경험한 경우 위험이 증가한다.[77][78]정상 출산 후 PTSD 유병률(사산 또는 주요 합병증 제외)은 6주 산후에서 2.8~5.6%로 추정되며,[79] 6개월 산후에서는 1.5%로 낮아진다.[79][80]PTSD의 증상은 출산 후 흔하게 나타나며, 6주에는 24~30.1%의 [79]유병률이 6개월에는 13.6%로 낮아진다.[81]긴급 출산은 또한 PTSD와 관련이 있다.[82]

유전학

PTSD에 대한 감수성이 유전적이라는 증거가 있다.PTSD의 분산의 약 30%는 유전학에서만 발생한다.[49]베트남에서 전투에 노출된 쌍둥이의 경우, PTSD와 함께 일란성(식별성) 쌍둥이를 갖는 것은 현기증(비식별성 쌍둥이)인 쌍둥이에 비해 동반성 쌍둥이의 PTSD 위험 증가와 관련이 있었다.[83]해마가 작은 여성들은 사전 발견에 근거한 외상 후 PTSD에 걸릴 가능성이 더 높다.[84]연구는 또한 PTSD가 다른 정신 질환에 공통적인 많은 유전적 영향을 공유한다는 것을 발견했다.공황과 일반화된 불안장애와 PTSD는 동일한 유전적 분산의 60%를 공유한다.알코올, 니코틴, 약물 의존도는 40% 이상의 유전적 유사성을 공유한다.[49]

이후의 PTSD 개발과 관련된 몇 가지 생물학적 지표가 확인되었다.초기 반응의 증가와 예비 결과만 있을 뿐, PTSD 개발의 위험성을 높이기 위해 더 작은 해마 부피가 가능한 바이오마커로 확인되었다.[85]또한, 한 연구는 백혈구가 더 많은 글루코코르티코이드 수용체를 가지고 있는 군인들이 트라우마를 겪은 후 PTSD에 더 쉽게 발병한다는 것을 발견했다.[86]

병리학

신경내분비학

외상성 질환이 뇌에 깊은 신경학적 패턴을 만드는 과잉반응 아드레날린 반응을 일으킬 때 PTSD 증상이 나타날 수 있다.이러한 패턴은 공포를 촉발한 사건이 발생한 후에도 오래 지속되어, 개인이 미래의 두려운 상황에 지나치게 반응하게 만들 수 있다.[24][87]외상 경험 동안, 높은 수준의 스트레스 호르몬은 PTSD의 발전을 위한 주요 요인이 될 수 있는 저체온성 활동을 억제한다.[88]

PTSD는 뇌와 신체에 생화학적 변화를 일으키는데, 이는 주요 우울증과 같은 다른 정신 질환과는 다르다.PTSD 진단을 받은 개인은 임상적 우울증을 진단받은 개인보다 덱사메타손 억제 테스트에 더 강력하게 반응한다.[89][90]

PTSD에 걸린 대부분의 사람들은 소변에서 코르티솔의 낮은 분비와 카테콜아민의 높은 분비를 보이며,[91] 결과적으로 노레피네프린/코르티솔 비율이 비진단 개인에 비해 높다.[92]이는 스트레스 요인에 노출된 후 카테콜아민과 코티솔 수치가 모두 상승하는 규범적 투쟁-비행 반응과는 대조적이다.[93]

뇌 카테콜아민 수치가 높고,[94] 코르티코트로핀-방출인자(CRF) 농도가 높다.[95][96]이와 함께, 이러한 발견은 저팔라믹-피티하수체-아드레날린(HPA) 에 이상이 있음을 시사한다.

공포의 유지에는 HPA 축, 로쿠스 코울레우스-노라드레날린계통, 변연계전두피질 사이의 연결부가 포함된 것으로 나타났다.스트레스에 대한 호르몬 반응을 조정하는 [97]HPA 축은 LC-노라드레날린 시스템을 활성화하며 트라우마의 여파로 발생하는 기억의 과잉 결합에 관여한다.[98]이러한 과잉 결합은 사람의 PTSD 발병 가능성을 높인다.편도체는 위협에 대한 대응으로 수행되는 위협 탐지 및 조건화되고 조건 없는 공포 대응의 책임이 있다.[49]

HPA 축은 스트레스에 대한 호르몬 반응을 조정하는 역할을 한다.[49]PTSD에서 덱사메타손에 대한 강한 코티솔 억제를 고려할 때, HPA 축 이상 현상은 글루코코르티코이드 수용체의 민감도 증가에 기인하여 코티솔의 강한 음성 피드백 억제에 의해 예측될 가능성이 있다.[99]PTSD는 과민성, 과민반응, 과민반응 HPA 축을 통한 대응을 두려워하는 부적응 학습 경로로 가정되어 왔다.[100]

코티솔 수치가 낮을 경우 개인에게 PTSD를 유발할 수 있다: 전쟁 트라우마에 이어 보스니아 헤르체고비나에서 복무하는 스웨덴 병사들은 전쟁 트라우마에 이어 PTSD 증상과 반응할 위험이 정상의 사전 서비스 레벨을 가진 병사들보다 높았다.[101]코티솔은 보통 스트레스 반응 후 동면상태를 회복하는 데 중요하기 때문에 코티솔이 낮은 외상 생존자들은 제대로 억제되지 못한 상태(즉, 더 길고 더 고통스러운 반응)를 경험하며 PTSD의 장을 마련한다고 생각된다.

로쿠스 코에룰루스-노라드레날린 체계는 공포기억의 과도한 통합을 매개한다고 생각된다.코티솔의 높은 수치는 노라드레날린 활동을 감소시키며, PTSD를 가진 사람들은 코티솔의 수치를 감소시키는 경향이 있기 때문에, PTSD를 가진 사람들은 외상성 스트레스에 대해 증가된 노라드레날린 반응을 규제할 수 없다고 제안되었다.[88]거슬리는 기억과 조건부 공포 반응은 관련 트리거에 대한 반응의 결과로 생각된다.뉴로펩타이드 Y(Neuropeptide Y, NPY)는 노레피네프린 방출을 줄이는 것으로 보고되었으며 동물 모델에서 항불안제 성질을 보이는 것으로 밝혀졌다.연구는 PTSD를 앓고 있는 사람들이 NPY의 감소된 수준을 보여주며, 그들의 불안 수준이 증가했음을 보여준다.[49]

다른 연구들은 PTSD를 앓는 사람들이 만성적으로 낮은 수준의 세로토닌을 가지고 있다는 것을 보여주는데, 이것은 불안, 반론, 자극성, 공격성, 자살성, 충동성과 같은 흔히 연관된 행동 증상에 기여한다.[102]세로토닌은 또한 글루코코르티코이드 생산의 안정화에 기여한다.

PTSD를 앓고 있는 사람의 도파민 수치는 증상에 기여할 수 있다: 낮은 수치는 무쾌감증, 무관심, 주의력 저하, 운동결손의 원인이 될 수 있다; 높은 수치는 정신병, 동요, 그리고 안절부절못의 원인이 될 수 있다.[102]

몇몇 연구들은 PTSD에서 갑상선 호르몬트리오도시로닌의 높은 농도를 설명했다.[103]이러한 종류의 2형 전위 적응은 카테콜아민 및 기타 스트레스 매개체에 대한 민감도를 증가시키는 데 기여할 수 있다.

노레피네프린 시스템에서 과민반응은 또한 높은 스트레스에 지속적으로 노출되어 야기될 수 있다.전두엽 피질에서 노레피네프린 수용체의 과활성화는 PTSD를 가진 사람들이 자주 경험하는 플래시백과 악몽과 연결될 수 있다.다른 노레피네프린 기능(현재 환경의 인식)의 감소는 뇌의 기억 메커니즘이 경험을 처리하는 것을 방해하며, 플래시백(flashback) 동안 사람이 경험하고 있는 감정은 현재 환경과 관련이 없다.[102]

의학계에서는 PTSD의 신경생물학에 대해 상당한 논란이 있다.2012년 한 리뷰는 코티솔 수치와 PTSD 사이의 명확한 관계를 보여주지 않았다.대부분의 보고서는 PTSD를 앓고 있는 사람들이 덱사메타손에 의한 코르티코트로핀 방출 호르몬 수치 상승, 기저 코티솔 수치 감소, HPA 축에 대한 음성 피드백 억제를 강화했다는 것을 보여준다.[49][104]

신경안내술

구조 MRI 연구의 메타 분석 결과, 총 뇌 용적, 두개내 용적, 해마, 인슐라 피질, 전측 정맥의 부피와 연관성이 발견되었다.[106]이 연구의 대부분은 베트남 전쟁에 노출된 사람들의 PTSD에서 비롯된다.[107][108]

PTSD를 앓고 있는 사람들은 감정의 경험과 조절에 관련된 영역인 등측 및 등측 전측두엽 피질 및 뇌전두피질에서 뇌의 활동을 감소시켰다.[109]

편도체는 감정적 기억, 특히 공포와 관련된 기억을 형성하는 데 강하게 관여한다.높은 스트레스를 받는 동안 공간과 시간, 기억의 정확한 맥락에 기억을 배치하는 것과 관련이 있는 해마는 억제된다.한 이론에 따르면 이 억제는 PTSD를 가진 사람들에게 영향을 줄 수 있는 기억의 원인일 수 있다.PTSD를 가진 사람이 외상 사건과 유사한 자극을 받았을 때, 신체는 그 기억이 그 사람의 기억 속에 제대로 기록되지 않았기 때문에 그 사건이 다시 일어나는 것으로 인식한다.[49][110]

PTSD의 편도체 중심 모델은 편도체가 특히 멸종하는 동안 내측 전전두피질과 해마에 의해 매우 많이 자극되고 불충분하게 조절된다고 제안한다.[111]이는 PTSD가 부족한 멸종 능력의 증후군이라는 해석과 일치한다.[111][112]

편도체의 기저핵(BLA)은 자극에 대한 조건부 반응과 조건부 반응 사이의 연관성을 비교하고 발전시키는 역할을 하며, 이는 PTSD에 존재하는 공포 조절을 초래한다.BLA는 편도체의 중심핵(CeA)을 활성화시켜 공포반응을 정교하게 한다(위협에 대한 행동반응과 상승기동반응 포함).mPFC(medial prefrontal cortex, mPFC)로부터의 내림 억제 입력은 BLA에서 CeA로의 전달을 조절하며, 이는 조건화된 공포 반응의 소멸에 역할을 한다는 가설을 세운다.[49]전체적으로 편도체는 PTSD에서 기능 신경영상화 메타분석에 의해 보고되지만, 사회불안장애나 공포증 장애보다 더 많은 이질적 성질이 있다.등부(대략 CeA)와 복부(대략 BLA) 클러스터를 비교했을 때, 등부(등부) 군집에서는 과잉 활동이 더 강하지만, 등부 군집에서는 저활동이 명백하다.그 구분은 공포와 관련된 요소뿐만 아니라 PTSD(CeA의 감소를 통해)의 위축된 감정을 설명할 수 있다.[113]

2007년 연구에서 PTSD를 가진 베트남전 참전용사들은 해마의 부피가 20% 감소하는 것을 보여주었는데, 해마는 그런 증상이 없었다.[114]이 발견은 1988년 에어쇼 비행기 추락사고로 정신적 충격을 받은 만성 PTSD 환자들(독일 람슈타인)에서는 재현되지 않았다.[115]

증거에 따르면 PTSD, 특히 아나다미드에서는 내생성 카나비노이드 수치가 감소하고, 보상하기 위해 카나비노이드 수용체(CB1)가 증가한다고 한다.[116]편도선 내 CB1 수용체 가용성 증가와 외상 생존자의 이상 위협 처리 및 과호흡 사이에는 연관성이 있는 것으로 보인다.

2020년 한 연구는 낮은 IQ가 PTSD를 개발하는 데 위험 요소라는 것을 시사한 이전 연구로부터의 결론에 대한 증거를 발견하지 못했다.[117]

진단

PTSD는 다음과 같은 이유로 진단하기 어려울 수 있다.

  • 대부분의 진단 기준의 주관적 특성(많은 정신 질환에 해당하지만)
  • 예를 들어, 장애 수당을 구하는 동안 또는 PTSD가 형사 판결에서[118] 완화 요소가 될 수 있는 경우 과대 보고의 가능성
  • 예를 들어, PTSD 진단이 특정 고용 기회를 방해할 수 있다는 오명, 자부심, 두려움 등의 과소 보고 가능성
  • 강박증, 일반화된 불안장애 등 다른 정신질환과 증상이 겹치는 [119]경우
  • 주요 우울증 및 일반화된 불안장애와 같은 기타 정신질환과의 연관성
  • 물질 사용 장애(PTSD와 동일한 징후와 증상 중 일부를 생성하는 경우가 많음)
  • 물질 사용 장애는 PTSD에 대한 취약성을 증가시키거나 PTSD 증상 또는 둘 모두를 악화시킬 수 있다.
  • PTSD는 물질 사용 장애의 발생 위험을 증가시킨다.
  • 문화적으로 증상의 차등적 표현([120]기피와 무감각증상, 괴로운 꿈, 체증상에 관한 것)

선별

DSM-5의 PTSD 체크리스트([121][122]PCL-5)와 DSM-5의 Primary Care PTSD 화면(PC-PTSD-5) 등 성인을 위한 PTSD 선별 기기가 다수 있다.[123]

어린이와 청소년과 함께 사용할 수 있는 선별 및 평가 기구도 여러 개 있다.여기에는 DSM-IV를 위한 아동 PTSD 증상 척도([124][125]CPSS), 아동 외상 선별 설문지 [126][127]및 UCLA 외상 후 스트레스 장애 반응 지수가 포함된다.[128][129]

이밖에도 매우 어린(만 6세 이하) 아동의 간병인을 위한 검진 및 평가 도구도 있다.여기에는 어린아이의 PTSD [130]화면, 어린아이의 PTSD 체크리스트,[130] 그리고 진단 유아 및 유치원 평가가 포함된다.[131]

평가

다중 방법 평가 접근방식을 포함한 증거 기반 평가 원칙은 PTSD 평가의 기초를 형성한다.[132][133][134]

진단 및 통계 설명서

PTSD는 DSM-IV에서는 불안장애로 분류되었으나, 이후 DSM-5에서는 "트라우마와 스트레스 요인 관련 장애"로 재분류되었다.[1]PTSD에 대한 DSM-5 진단 기준은 재실험, 회피, 인지/무드의 음성 변경, 흥분 및 반응성의 변경 등 4가지 증상 클러스터를 포함한다.[1][3]

국제질병분류

국제질병분류 10(ICD-10)은 PTSD를 "심각한 스트레스와 조절장애에 대한 반응"[135]에 따라 분류한다.PTSD에 대한 ICD-10 기준은 재실험, 회피, 반응성 증가 또는 이벤트와 관련된 특정 세부사항의 회수 불능을 포함한다.[135]

PTSD에 대한 ICD-11 진단 설명에는 세 가지 구성 요소 또는 증상 그룹 (1) 재검사, (2) 회피, (3) 위협 의식 고조가 포함된다.[136][137]ICD-11은 더 이상 그 충격적인 사건에 대한 언어적 생각을 증상으로 포함하지 않는다.[137]ICD-10 또는 DSM-5에 비해 ICD-11을 사용하여 진단된 PTSD의 비율이 더 낮을 것으로 예상된다.[137] ICD-11은 또한 복합 외상 후 스트레스 장애(CPTSD)를 가진 별개의 그룹을 식별할 것을 제안한다. 복합 외상 후 스트레스 장애(CPTSD)는 PTSD보다 여러 번 또는 지속적 트라우마를 더 자주 경험하고 기능 장애가 있다.[137]

차등진단

PTSD 진단을 위해서는 극도의 스트레스 요인에 노출되어 있어야 한다.모든 스트레스 요인은 조정 장애를 진단할 수 있으며 스트레스 요인에 대한 적절한 진단이며 PTSD의 기준을 충족하지 못하는 증상 패턴이다.

급성 스트레스 장애의 증상 패턴은 외상 후 4주 이내에 발생하여 해결되어야 한다.증상이 더 오래 지속되고 증상 패턴이 PTSD의 특성에 맞으면 진단이 변경될 수 있다.[23]

강박장애는 반복되지만 특정 외상 사건과 관련이 없는 침입적 사고에 대해 진단될 수 있다.[23]

생존자들은 생존 가능성이 없는 장기적이고 반복적인 외상 후 스트레스 장애에 걸릴 수 있다.[138]이것은 단일 외상 사건보다는 외상 층의 결과로 발생하며, 일관성 있는 자아감각 상실과 같은 추가 증상학을 포함한다.[139]

예방

인지 행동 요법에 대한 초기 접근에서 약간의 편익이 관찰되어 왔다.심각한 사고 스트레스 관리는 PTSD를 예방하는 수단으로 제안되어 왔지만, 이후 연구는 그것이 부정적인 결과를 낳을 가능성을 제시한다.[140][141]2019년 코크란 리뷰에서는 '모든 사람에게 제공되는 개입의 사용을 뒷받침할 어떤 증거도 발견하지 못했다' '복수의 세션 개입은 일부 개인에게 개입하지 않는 것보다 더 나쁜 결과를 초래할 수 있다'고 밝혔다.[142]세계보건기구(WHO)는 급성 스트레스(증상 1개월 미만 지속)에 벤조디아제핀항우울제를 사용하지 말 것을 권고하고 있다.[143]프로프라놀올, 에스시탈로프람, 테마제팜 또는 가바펜틴을 지지하는 증거가 제한적이거나 전혀 없지만, 일부 증거는 성인의 예방에 하이드로코르티손의 사용을 지지한다.[144]

심리학 설명회

외상 노출 개인들은 PTSD를 예방하기 위해 심리적인 결과 보고라는 치료를 받는 경우가 많은데, PTSD는 개인들이 직접 사건에 직면하고 자신의 감정을 상담사와 공유할 수 있도록 하고, 사건에 대한 기억을 구조화하는 데 도움을 주기 위한 인터뷰로 구성된다.[145]그러나, 몇몇 메타 분석가들은 심리적인 보고가 도움이 되지 않고 잠재적으로 해로울 수 있다는 것을 발견한다.[145][146][147]이것은 단일 세션 보고와 다중 세션 개입 모두에 적용된다.[142]2017년 현재 미국심리학회는 심리학적 보고가 "연구지원/치료가 잠재적으로 유해하다"고 평가했다.[148]

위험 대상 개입

위험 대상 개입은 특정한 형태 형성적 정보나 사건을 완화시키려 하는 개입이다.그것은 정상 행동 모델링, 직무에 대한 지시 또는 이벤트에 대한 정보를 제공하는 것을 목표로 할 수 있다.[149][150]

관리

연구 검토 결과 복합요법(심리치료와 약리치료)은 심리치료만 해도 효과가 없다는 결과가 나왔다.[14]

상담

가장 강력한 증거를 가진 접근방법으로는 장기 노출치료,[151] 인지처리치료, 안구운동감소 재처리(EMDR) 등 행동 및 인지행동요법이 있다.[152][153][154]간단한 절연심리치료(BEP), 서술노출치료(NET), 서면노출치료에 대한 몇 가지 증거가 있다.[155][156]

2019년 코크란 리뷰는 PTSD 치료를 위한 개인 및 단체 치료와 비교해서 부부 및 가족 치료법을 평가했다.[157]실질적인 편익이 도출되었는지 판단하기에는 부부 치료법에 대한 연구가 너무 적었지만 예비 RCTs는 부부 치료법이 PTSD 증상을 줄이는 데 도움이 될 수 있다고 제안했다.[157]

EMDR과 인지행동요법(CBT)의 메타분석적 비교 결과, PTSD를 치료하는 효과 면에서 두 프로토콜 모두 구별할 수 없는 것으로 나타났지만, "치료 결과에 대한 EMDR의 눈 움직임 구성요소의 기여"는 불분명하다.[158]어린이와 청소년의 메타 분석에서도 EMDR이 CBT만큼 효과적이라는 것을 발견했다.[159]

PTSD를 앓고 있는 아이들은 무료 진료소보다 학교에서 치료를 받을 가능성이 훨씬 더 높다.[160]

인지행동요법

이 도표는 감정, 생각, 행동 모두가 서로에게 어떤 영향을 미치는지 그리고 있다.가운데 삼각형은 모든 인간의 핵심신앙을 자기, 다른 사람, 미래 등 세 가지 범주로 요약할 수 있다는 CBT의 신조를 나타낸다.

CBT는 부정적인 감정을 책임지는 사고나 행동, 혹은 둘 다의 패턴을 변화시킴으로써 사람이 느끼고 행동하는 방식을 바꾸려고 한다.2018년 체계적 검토 결과 PTSD 및 우울증 증상 감소와 PTSD 진단 상실에 효과적인 CBT 노출 요법을 뒷받침하는 증거가 높은 것으로 나타났다.[161]CBT는 PTSD에 효과적인 치료법임이 입증되었으며 현재 미국 국방부가 PTSD에 대한 관리 표준으로 간주하고 있다.[162][163]CBT에서 개인은 자신을 두렵게 하거나 화나게 하는 생각을 식별하고 덜 괴로운 생각으로 대체하는 것을 배운다.그 목적은 사건에 대한 어떤 생각이 어떻게 PTSD와 관련된 스트레스를 유발하는지 이해하는 것이다.[164][165]인터넷 기반 형식의 CBT 제공은 2018년 코크란 리뷰에서도 연구되었다.이 검토는 인터넷 기반 설정에 대해 대면과 유사한 이로운 효과를 발견했지만, 검토된 재판 횟수가 적어서 증거의 질이 낮았다.[166]

노출 치료는 외상 생존자들이 외상 기억의 거주화와 성공적인 감정 처리를 용이하게 하기 위해 고통스러운 외상 관련 기억과 상기 사항을 다시 경험할 수 있도록 돕는 인지 행동 치료의[167] 일종이다.대부분의 노출 치료 프로그램에는 외상 기억과의 상상의 대립과 외상 주의사항에 대한 실제 노출이 모두 포함된다. 이 치료 방법 모달리티는 임상 증거에 의해 잘 뒷받침된다.[citation needed]노출 기반 치료법의 성공으로 노출이 PTSD 치료에 필요한 성분인지에 대한 의문이 제기되고 있다.[168]일부 조직은[which?] 노출의 필요성을 지지했다.[169][170]미국 보훈처는 PTSD로 미국 참전용사들을 더 잘 치료하기 위해 장기간 노출 치료와[171] 인지 처리[172] 치료에서 정신 건강 치료 요원을 적극적으로 양성해 왔다.

문맥에 기반한 제3세대 행동 치료법에 대한 최근의 연구는 그것들이 더 잘 검증된 치료법들 중 몇 가지에 필적할 만한 결과를 낼 수 있다는 것을 시사한다.[173]이러한 치료 방법들 중 다수는 노출의[174] 중요한 요소를 가지고 있으며 PTSD의 주요 문제와 함께 발생하는 우울증 증상을 치료하는데 성공했음을 입증했다.[175]

눈 움직임 감압 및 재처리

안구운동감소화 및 재처리(EMDR)는 프랑신 샤피로가 개발하고 연구하는 심리치료의 한 형태다.[176]그녀는 스스로 기억을 교란시킬 생각을 하고 있을 때, 그녀의 눈이 빠르게 움직이고 있다는 것을 알아차렸었다.그녀가 생각하면서 눈동작을 억제했을 때, 그 생각들은 덜 괴로웠다.[176]

2002년에 샤피로와 맥스필드는 적응 정보 처리라고 불리는 이것이 왜 효과가 있을지에 대한 이론을 발표했다.[177]이 이론은 눈의 움직임을 기억의 정서적 처리를 용이하게 하는데 사용할 수 있다고 제안하고, 그 사람의 기억을 좀 더 적응적인 정보에 참여하도록 변화시킨다.[178]치료사는 자발적인 빠른 눈 움직임을 시작하는 반면, 그 사람은 특정한 트라우마에 대한 기억, 감정 또는 생각에 초점을 맞춘다.[43][179]치료사들은 손의 움직임을 이용하여 눈을 앞뒤로 움직이게 하지만 손뼉을 치거나 소리를 내는 것도 사용할 수 있다.[43]EMDR은 노출(외상 사고 재방문), 인지 과정 작업 및 이완/자율 모니터링을 결합하여 인지 행동 요법과 매우 유사하다.[43]그러나, 경험에 대해 이야기하기 보다는 생각하라는 방식으로 노출되는 것은 EMDR의 더 중요한 구별 요소 중 하나로 부각되었다.[180]

어린이와 청소년뿐만 아니라 성인에서도[181] 4~8주간의 EMDR에 대한 소규모 통제 실험이 여러 차례 있었다.[179]2018년 체계적인 리뷰 업데이트에 따르면 EMDR의 효능을 뒷받침할 수 있는 증거들이 중간 정도 강도에 있다.[161] "PTSD 증상 감소, 진단 손실, 우울 증상 감소"EMDR은 단기적으로 성인 2명 중 1명이 더 이상 PTSD의 기준을 충족하지 못할 정도로 PTSD 증상을 감소시켰지만, 이러한 실험에 참여하는 사람의 수는 적었기 때문에 추가 연구가 있을 때까지 결과를 신중하게 해석해야 한다.[181]EMDR이 성인의 PTSD를 제거할 수 있는지 여부를 알 만한 충분한 증거가 없었다.[181]아동·청소년의 경우, 최근 메타NSUE를 이용한 무작위 제어 시험의 메타 분석 결과, EMDR이 최소한 CBT만큼 효과적이며, 대기자 명단이나 위약보다 우수하다는 사실이 밝혀졌다.[159]EMDR이 우울증을 예방할 수 있다는 증거가 있었다.[181]EMDR을 다른 심리치료나 약물치료와 비교한 연구는 없었다.[181]부작용은 대체로 없었다.[181]다른 유형의 트라우마 사건(사고, 폭행, 전쟁 등)을 경험한 사람에 비해 성폭행 전력이 있는 여성에게 혜택이 더 컸다.EMDR이 아동·청소년의 재실험 증상을 개선시킬 수 있다는 증거는 적지만 EMDR은 다른 PTSD 증상이나 불안, 우울증을 개선하지 못하는 것으로 나타났다.[179]

이 치료의 눈 움직임 요소는 유익성을 위해 중요하지 않을 수 있다.[43][178]그동안 안구동작 EMDR 대 안구동작 EMDR에 대한 주요 고품질 무작위 임상시험이 없었던 만큼 실효성 논란은 계속될 것으로 보인다.[180]2013년 발간된 메타 분석의 저자들은 "눈동작 요법으로 치료받은 사람들이 눈동작 없는 치료를 받은 사람들보다 외상 후 스트레스 장애의 증상이 더 많이 개선된 것을 발견했다"고 밝혔다.둘째로, 실험실 연구에서 우리는 기억을 속상하게 하는 생각과 눈의 움직임을 용이하게 하는 작업을 동시에 하는 것이 속상한 기억과 관련된 생생함과 괴로움을 감소시킨다는 증거를 발견했다."[153]

대인관계 심리치료

특히 사회적 지원을 포함하는 다른 접근법도 중요할 수 있다.[182][183]대인관계 심리치료의[184] 공개적인 실험은 노출을 사용하지 않고 PTSD 증상으로 인한 높은 완화율을 보고했다.[185]현재 뉴욕에서 NIMH가 후원하는 NIMH가 시행하고 있는 재판은 대인관계 심리치료, 장기 노출치료, 이완치료 등을 비교하는 것이다.[186][full citation needed][187][188]

약물

많은 약들이 그들의 사용을 뒷받침할 충분한 증거를 가지고 있지 않지만, 네 가지 (sertraline, fluoxetine, paroxetine, venlafaxine)는 위약에 비해 작거나 적은 편익을 가지고 있는 것으로 나타났다.[16]많은 약물치료와 함께 치료 후 남은 PTSD 증상은 예외가 아니라 규칙이다.[189]

항우울제

선택적 세로토닌 재흡수 억제제(SSRIs)와 세로토닌-노레피네프린 재흡수 억제제(SNRIs)는 PTSD 증상에 어느 정도 도움이 될 수 있다.[16][190][191]삼발성 항우울제는 똑같이 효과가 있지만 잘 견디지 못한다.[192]증거는 Sertraline, fluoxetine, paroxetine, venlafaxine으로 작거나 다소 개선된 것을 뒷받침한다.[16][193]따라서 이 4가지 약물은 PTSD를 위한 1차 의약품으로 간주된다.[190][4]

벤조디아제핀

벤조디아제핀은 유익성의 증거가 부족하고 PTSD 증상이 악화될 위험이 있어 PTSD 치료에 권장되지 않는다.[194]어떤 저자들은 벤조디아제핀의 사용이 급성 스트레스에 억제된다고 믿는다. 이 약의 그룹은 분열을 일으킬 수 있기 때문이다.[195]그런데도 단기불안이나 불면증에 주의해서 벤조디아제핀을 사용하는 사람도 있다.[196][197][198]벤조디아제핀은 급성 불안감을 완화시킬 수 있지만 PTSD 개발을 중단할 수 있고 실제로 PTSD 발병 위험을 2~5배 높일 수 있다는 일관된 증거가 없다.[15]또한, 벤조디아제핀은 심리치료적 개입의 효과를 감소시킬 수 있으며, 벤조디아제핀이 실제로 PTSD의 발전과 연대기에 기여할 수 있다는 증거가 있다.이미 PTSD를 앓고 있는 사람들에게 벤조디아제핀은 심리치료 결과를 악화시키고 공격성, 우울증(자살성 포함), 약물 사용을 유발하거나 악화시킴으로써 병의 진행을 악화시키고 연장시킬 수 있다.[15]단점으로는 벤조디아제핀 의존성, 내성(즉, 시간이 지남에 따라 소멸되는 단기 유익성) 및 금단 증후군이 발생할 위험이 있으며, PTSD를 가진 개인(알코올 또는 약물 오용의 이력이 없는 개인도)이 벤조디아제핀을 남용할 위험이 증가한다는 점이 있다.[4][199]PTSD에 더 큰 효능이 있고 위험이 덜한 여러 다른 치료들 때문에(예: 장기 노출, 인지 처리 요법, 눈 움직임 감응 및 재처리, 인지 구조 조정 요법, 외상 중심 인지 행동 요법, 간략한 절충 심리 치료법, 서술 요법, 스트레스 접종 훈련, 세로토늄)에르그틱 항우울제, 아드레날린 억제제, 항정신병 약물 및 심지어 항경련제), 벤조디아제핀은 다른 치료 선택사항이 모두 소진될 때까지 상대적으로 억제된 것으로 간주해야 한다.[13][200]벤조디아제핀은 가장 심각한 경우 더 빨리 사용해야 한다고 주장하는 사람들에게, (자살성, 공격성 및 범죄와 관련됨) 불능의 부정적 위험과 확정적인 효과적인 치료를 지연하거나 억제하는 임상적 위험은 다른 대체 치료법을 선호하게 만든다(예: 입원, 거주지,zation, 집중 외래환자, 변증법 행동요법, 그리고 trazodone, mirtazapine, amitripytline, doxepin, prazosin, propranolol, 관파신, clonidine, ketiapine, alanzapine, valpropropentin)[4][200][201]과 같은 다른 빠른 반응의 진정제.

프라조신

알파-1 아드레날린 적수인 프라조신은 악몽을 줄이기 위해 PTSD를 가진 참전용사들에게 사용되었다.연구는 증상 개선, 적절한 복용량, 그리고 이 모집단의 효능의 가변성을 보여준다.[202][203][22]

글루코코르티코이드

글루코코르티코이드는 PTSD의 특성을 나타내는 확장된 스트레스 반응에 의해 야기되는 신경 탈생으로부터 보호하기 위한 단기 치료에 유용할 수 있지만, 장기적 사용은 실제로 신경 탈생을 촉진시킬 수도 있다.[204]

카나비노이드

대마초는 현재 카나비노이드에 대한 치료 효능을 입증하는 과학적 증거가 존재하지 않기 때문에 PTSD 치료제로 권장되지 않는다.[205][206][b]그러나 PTSD를 앓고 있는 미국 참전용사들 사이에서는 대마초나 파생상품의 사용이 널리 퍼져 있다.[207]

카나비노이드 나빌론은 때때로 PTSD의 악몽에 사용된다.비록 일부 단기적인 유익성이 나타났지만, 부작용은 흔하고 효능을 판단하기 위해 충분히 연구되지 않았다.[208]점점 더 많은 주에서 PTSD 치료를 위한 의료용 대마초 사용을 허용하고 합법화했다.[209]

기타

운동, 운동, 신체 활동

신체 활동은 사람들의[210] 심리적, 육체적 건강에 영향을 미칠 수 있다.[211]미국 국립 PTSD 센터는 감정을 어지럽히는 것으로부터 주의를 산만하게 하고 자존감을 형성하며 다시 통제할 수 있는 감정을 증가시키는 방법으로 적당한 운동을 권장하고 있다.그들은 운동 프로그램을 시작하기 전에 의사와 상의할 것을 권한다.[212]

어린이 놀이치료

놀이는 어린이들이 내면의 생각과 외부 세계를 연결시켜 실제 경험을 추상적인 생각과 연결시켜주는 데 도움을 주는 것으로 생각된다.[213]반복적인 놀이는 또한 아이가 외상 사건을 다시 경험하는 한 가지 방법이 될 수 있으며, 그것은 어린이나 젊은 사람에게 트라우마의 증상일 수 있다.[214]흔히 쓰이지만 놀이치료를 받고 받지 않는 아이들의 집단에서 결과를 비교하는 연구가 충분하지 않아 놀이치료의 효과는 아직 파악되지 않고 있다.[43][213]

군사 프로그램

이라크아프가니스탄 전쟁의 많은 참전용사들은 신체적, 정서적, 관계적 장애에 직면해 있다.이에 미 해병대는 특히 배우자와 사랑하는 사람들과의 관계에서 그들이 더 잘 소통하고 상대방이 어떤 일을 겪었는지 이해할 수 있도록 민간인 생활에 재조정하는 데 도움을 주는 프로그램을 마련했다.[215]WRAIR(Walter Reed Army Institute of Research)는 서비스 구성원들이 PTSD 및 관련 문제를 피하거나 개선하도록 돕기 위해 배틀마인드 프로그램을 개발했다.부상당한 전사 프로젝트는 미국 보훈처와 제휴하여 PTSD 치료 센터의 국가 건강 시스템인 전사 돌봄 네트워크를 만들었다.[216][217]

악몽

인구에서 발견된 PTSD의 비율에 대한 논쟁이 있지만, 1997년과 2013년 사이에 PTSD를 정의하는 데 사용된 진단과 기준의 변화에도 불구하고, 역학률은 크게 변하지 않았다.[220][221]PSM-5가 2013년에야 도입되었기 때문에 PTSD의 역학에 관한 현재 신뢰할 수 있는 데이터의 대부분은 DSM-IV 기준에 근거한다.

군사 전투

미국 보훈처는 베트남전 참전용사 83만명이 PTSD 증세를 겪은 것으로 추산하고 있다.[227]국가베트남 재향군인 정비 연구(NVRS)는 연구 당시 베트남 재향군인의 15%, 여성 재향군인의 9%가 PTSD를 갖고 있는 것으로 나타났다.PTSD의 수명 유병률은 남성이 31%, 여성이 27%로 나타났다.NVVRS 데이터를 재분석한 결과, Matsunaga 베트남 참전용사 프로젝트, Schnurr, Lunney, Sengupta 및 Waelde의 데이터를 분석한 결과 NVVVRS 데이터의 초기 분석과는 달리 대다수의 베트남 참전용사가 PTSD 증상(단, 장애 자체는 아님)으로 고통 받는다는 것을 발견했다.5명 중 4명은 베트남이후 20~25년 후 인터뷰했을 때 최근의 증상을 보고했다.[228]

인재

영국에는 다양한 자선 단체와 민간 생활로의 재조정에서 참전용사들을 돕는 봉사 단체들이 있다.왕립 영국 군단과 더 최근에 설립된 Help for Heroes는 영국의 더 유명한 퇴역군인 단체들 중 두 곳으로 수년에 걸쳐 퇴역군인들을 위해 적극적으로 옹호해왔다.국민건강보험공단이 정신건강 문제에 대처하는 데 충분한 성과를 거두지 못했고 대신 '전투 스트레스'와 같은 자선단체에 대해 베테랑들이 '덤핑'하고 있다는 일부 논란이 있었다.[242][243]

역사

1952년판 DSM-I에는 PTSD의 현대적 정의 및 이해와 유사한 "총체 스트레스 반응"의 진단이 포함되어 있다.[248]총 스트레스 반응은 엄청난 스트레스의 조건에 대한 반응으로서 압도적인 공포를 다루기 위해 확립된 반응 패턴을 사용하는 정상적인 성격으로 정의된다.[249]그 진단은 "시민적 재난"뿐만 아니라 전투에 대한 조건과 관련된 언어를 포함한다.[249]

DSM-III에 이 용어를 추가한 것은 베트남전 미군 참전용사들의 경험과 여건에 크게 영향을 받았다.[253]베트남 전쟁과의 연관성 때문에 PTSD는 철도 척추, 스트레스 증후군, 향수, 군인의 심장, 조개 충격, 전투 피로, 전투 스트레스 반응, 외상성 전쟁 신경증 등 많은 역사적 전쟁 시간 진단과 동의어가 되었다.[254][255]이 용어들 중 일부는 19세기로 거슬러 올라가는데, 이는 이 조건의 보편적 성격을 나타낸다.비슷한 맥락에서, 정신과 의사 조나단 셰이크는 1597년경에 쓰여진 윌리엄 셰익스피어 극의 헨리 4세, 파트 1 (막 2,장 3, 줄 40–62[256])에서 퍼시 부인독백은 PTSD의 증상 별자리에 대한 비정상적으로 정확한 묘사를 나타낸다고 제안했다.[257]

리서치

PTSD에 관한 대부분의 지식은 고소득 국가의 연구로부터 온다.[269]

연구원들은 불안과 스트레스와 관련된 질병에 대한 더 나은 치료법을 찾기를 바라는 많은 실험적인 FAAH와 MAGL 억제 약물을 조사하고 있다.[273]2016년 FAAH 금지약물 BIA 10-2474가 역효과로 프랑스에서 인간 실험에서 철회됐다.[274]
