코박신(Cobaxin, BBV152로 코디네이트됨)은 인도의학연구회 - 국립바이올로지연구소와 공동으로 바랏바이오텍이 개발한 비활성화된 바이러스 기반 COVID-19 백신이다.

2021년 10월 현재 인도에서는 1억160만 명이 코박신을 받았다.[1]세계보건기구(WHO)는 2021년 11월 3일 긴급 사용을 위한 백신을 검증했다.[2][3]코박신은 2022년 1월 31일 현재 13개국에서 긴급 사용승인을 받았다.

의학적 용법

효과

백신은 일반적으로 추정치가 50%이고 신뢰 구간이 95%인 30% 이하인 경우 유효하다고 간주된다.[5]효과는 일반적으로 시간이 지남에 따라 서서히 감소할 것으로 예상된다.[6]

효능

2만5798명이 참여한 임상3상 임상3상에서 백신이 64%(95%), 증상질환 78%(65~86%), 중증질환 93%(57~100%), 델타변종 65%(33~83%)로 나타났다.[8]

제조업

비활성화된 백신으로서, 코박신은 비활성화된 소아마비 백신과 유사한 더 전통적인 기술을 사용한다.처음에, SARS-CoV-2의 샘플은 인도의 국립 비뇨기과 연구소에 의해 격리되었고 베로 세포를 사용하여 많은 양의 바이러스를 재배하는데 사용되었다.이때부터 바이러스는 베타 프로피올락톤에 담가 유전자에 결합시켜 비활성화시키는 한편 다른 바이러스 입자는 그대로 둔다.그 결과 활성화되는 바이러스는 알루미늄 기반 보조 바이러스 알하이드로시킴-II와 혼합된다.[9][10]

백신 지원자는 약 3억개의 선량을 전달할 수 있는 능력을 가진 바랏 바이오텍의 사내 베로 세포 제조 플랫폼으로[11] 생산된다.[12]이 회사는 하이데라바드의 게놈밸리 시설에 제2 공장을 세워 코박신을 만드는 작업을 진행 중이다. 회사는 오디샤 정부와 협력하여 부바네즈워오디샤 바이오테크 공원에 또 다른 시설을 설립하여 2022년 6월까지 코박신 생산을 개시할 예정이다.[13][14]이 외에도 코박신 제조에 대한 글로벌 제휴도 모색하고 있다.[15]

오쿠젠은 2020년 12월 미국 시장에서 코박신을 공동 개발하고 독점 상용화하기 위해 바라트바이오텍과 파트너십을 체결했고,[16][17] 2021년 6월 캐나다를 커버하기 위해 파트너십을 확대했다.[18]2021년 1월 프리사 메드는 바랏 바이오테크놀로지사와 코박신을 브라질에 공급하기로 협약을 맺었다.[19]

역사

임상시험

1단계 및 2단계 시험

2020년 5월 인도 의학 연구회의(ICMR) 국립 바이러스학 연구소는 완전 토종 COVID-19 백신을 개발하기 위해 바이러스 변종을 승인하고 제공했다.[23][24]2020년 6월 인도 정부의 마약 통제국(DCGI)으로부터 BBV152라는 이름의 개발 COVID-19 백신의 1단계와 2단계 인간 실험에 대한 허가를 받았다.[25]인도 1·2단계 의학연구협의회가 선정한 백신 후보 임상시험은 무작위, 이중 블라인드, 위약 제어 임상시험으로 총 12개소였다.[26][27][28]

2021년 1월, The Lancet에 1단계 시험 결과를 공표했다.[29]2021년 3월 8일, 2단계 결과가 The Lancet에 발표되었다.연구는 2단계 실험이 1단계와 투여 체계의 차이로 인해 면역 반응이 더 높고 T세포 반응을 유도했다는 것을 보여주었다.2단계에서의 선량은 1단계에서의 2주가 아닌 4주 간격으로 주어졌다.백신의 중성화 반응은 2단계에서 현저히 높게 나타났다.[30]

3단계 시험

코박신은 2020년 11월 1단계와 2단계를 완료한 후 3단계 인간실험을[31] 실시할 수 있는 승인을 받았다.[32]18세 이상 자원봉사자 중 무작위적이고 이중 맹목적이며 위약 조절이 가능한 연구는 11월 25일에 시작되었고 인도의 22개 사이트에서 약 26,000명의 자원봉사자들이 참여했다.[33][34][35]3단계 재판의 거부율은 1단계와 2단계보다 훨씬 높았다.그 결과, 12월 22일까지 모집된 자원봉사자는 1만 3천 명에 불과했고, 1월 5일까지는 2만 3천 명으로 늘어났다.[36][37]

미국 면역 결핍 및 부스터 단계 II/III 시험

2021년 6월 바라트바이오텍은 백신의 실제 효능을 평가하기 위한 4단계 실험의 시작을 발표했다.[43]코박신과 코비시드의 뉴델리 병원 맥스 그룹의 헬스케어 근로자에 대한 효과와 주저성 연구가 시행 중이다.[44]

미성년자 재판

2021년 5월 인도 마약통제국(DCGI)은 2~18세 연령대에서 임상시험을 승인했다.[45]이 실험은 AIMS 델리와 파트나에서 실시된다.[46]무려 54명의 아이들이 AIMS 파트나에 등록했다.[47]임상시험 데이터에 따라 총 525명의 참가자가 연구에 등록되어 있다.[48]

변형

2020년 12월 영국에서 알파 변종 또는 혈통 B.1.1.7이 확인되었다.[49]이 변종에 대한 시험관내 연구가 수행되었으며 예비 결과 코박신이 이 변종을 중화시키는데 효과적임을 알 수 있다.[50]

2021년 4월 인도의학연구회는 이 백신이 혈통 B.1.617을 중화시키는 유망한 결과를 보였다고 보고했다.[51][52]

2021년 5월 인도 국립바이올로지연구소(NIV) 과학자들의 공동 조사 결과, 이 백신은 제타 변종이나 혈통 P.2(이전에는 B.1.1.28)를 중화시키는 데 효과가 있는 것으로 밝혀졌다.[53]

바랏 바이오텍은 2020년 12월 6일 인도 마약통제국(DCGI)에 긴급 사용 허가를 신청했다.[57]인도 세럼연구소화이저에 이어 세 번째 기업이다.[58]

2021년 1월 2일 중앙의약품표준관리기구(CDSCO)가 허가를 권고했는데,[59] 다음날 허가됐다.[60]코박신은 "임상 시험 모드"로 사용되어야 했다. 즉, 공공 예방접종 추진은 그 자체로 개방 라벨의 단일 암 임상시험이 되어야 했다.[61]효용성과 안전에 관한 3단계 시험 자료를 고려하지 않고 허가된 이 긴급 승인은 광범위한 비판을 받았다.[62][63][64]2021년 10월 12일, 바라트바이오텍의 코박신은 2세에서 18세 사이의 어린이들에게 사용 허가를 받았다.[65]

다른 나라들

이 백신은 또한 이란과 짐바브웨에서 비상용으로 사용되도록 승인되었다.[66][67]네팔은 2021년 3월 19일 코박신을 허가했다.[68]4월 7일, 멕시코는 코박신에게 긴급 허가를 내주었다.[69]2021년 4월 19일 필리핀은 코박신에게 EUA를 승인했다.[70]또한 코박신은 과테말라, 니카라과, 가이아나, 베네수엘라, 보츠와나에서 EUA를 승인받았다.[71]

지난 3월 31일 브라질 보건 규제 기관인 안비사는 바랏 바이오테크놀로지의 코박신 공급 신청을 제조업 규범에 맞지 않는다는 이유로 거절했다.바라트바이오텍은 요구사항을 충족한 후 재신청하겠다고 밝혔다.[72]안비사는 지난 6월 4일 코박신의 예외적인 수입을 승인해 건강한 성인 위주로 제한하고, 부작용 통제와 감독을 통해 위험을 관리하기 위해 국내 인구의 1%에 불과한 조건으로 제한했다.안비사는 불완전한 3단계 연구, 국제적 합의를 충족시키기 위해 60일이 되어야 하는 45일간의 짧은 추적 연구, 그리고 자가면역 질환의 발병 가능성을 높일 수 있는 새로운 이미다주콜린 보조제 등을 주요 관심사로 꼽았다.[73][74][75]6월 30일, 브라질 감독당국은 이 거래를 중단시켰고, 연방 검사들에 의해 부정행위 혐의를 조사하기 위한 조사가 개시되었다.[76]안비사는 7월[77] 26일 진행 중인 백신의 임상시험을 취소하고 7월 27일 임시허가와 수입·유통허가를 중단했다.[78]

모리셔스는 2021년 3월 18일 코박신 첫 상업 공급을 받았다.[79]

2021년 3월 29일 파라과이는 코박신 10만 잔을 받았다.[80]

아르헨티나는 2021년 6월 코박신 1000만 t을 사들여 시민들에게 관리하기로 합의했다.[81]
