COVID-19 범유행(coronavirus pandemic)은 중증 급성 호흡기 증후군 코로나바이러스 2(SARS-CoV-2)에 의해 발생하는 코로나바이러스 질병 2019(COVID-19)의 전 세계적인 범유행입니다. 신종 바이러스는 2019년 12월 중국 우한시에서 발생한 발병에서 처음 확인된 후 2020년 초 아시아의 다른 지역으로 확산되었습니다. 세계보건기구(WHO)는 2020년 1월 30일 발병을 국제적 관심 공중보건 비상사태(PHEIC)로 선포하고, 2020년 3월 11일 발병이 팬데믹이 되었다고 평가했습니다.[3] WHO는 2023년 5월 5일 PHEIC을 종료했습니다.[4] 2024년 3월 8일 현재 팬데믹으로 인해 7,033,430명의[5] 사망자가 확인되었으며, 역사상 가장 치명적인 전염병전염병 목록에서 5위를 차지했습니다.

코로나19 증상무증상부터 치명적이지만 발열, 인후통, 야행성 기침, 피로감 등이 가장 많이 발생합니다. 바이러스의 전파는 종종 공기입자를 통해 이루어집니다. 돌연변이는 다양한 정도의 감염성과 독성을 가진 많은 균주(변이체)를 생성했습니다.[9]

코로나19 백신은 2020년 12월부터 여러 나라에 널리 보급되었습니다. 치료법에는 새로운 항바이러스제와 증상 조절이 포함됩니다. 공중 보건 비상 사태 동안 일반적인 완화 조치에는 여행 제한, 봉쇄, 영업 제한 및 폐쇄, 작업장 위험 통제, 마스크 의무화, 검역, 검사 시스템 및 감염자의 접촉 추적이 포함되었습니다.

배경

SARS-CoV-2는 박쥐 코로나바이러스,[25] 판골린 코로나바이러스,[26][27] SARS-CoV와 밀접한 관련이 있는 바이러스입니다.[28] 2019년 12월 중국 후베이성 우한에서 처음으로 알려진 발병(2019-2020년 중국 본토 코로나19 발생)이 시작되었습니다. 많은 초기 사례들이 그곳의 화난수산물도매시장을 방문했던 사람들과 연관되어 있었지만,[29][30][31] 사람 간 전염이 더 일찍 시작되었을 가능성이 있습니다.[32][33]

과학적 합의는 이 바이러스가 박쥐나 다른 밀접한 관련이 있는 포유동물로부터 인수공통전염병의 기원일 가능성이 가장 높다는 것입니다.[32][34][35] 실험실 유출설을 비롯한 바이러스의 기원에 대한 논란은 특히 미국과 중국 사이의 지정학적 분열을 고조시켰습니다.[36]

공식적인 '사례' 수는 코로나19 검사를 받고 공식 프로토콜에 따라 검사 결과 양성으로 확인된 사람의 수를 의미합니다.[45][46] 샘플링 편향의 효과로 인해 무작위 샘플에서 외삽하여 더 정확한 수를 얻는 연구에서는 총 감염이 보고된 사례 수를 상당히 초과한다는 것이 일관되게 발견되었습니다.[47][48] 초기에 많은 국가에서 가벼운 증상만 있는 사람은 검사하지 않는다는 공식 정책이 있었습니다.[49][50] 중증 질환의 가장 강력한 위험 요소는 비만, 당뇨병 합병증, 불안 장애 및 총 질환 수입니다.[51]

COVID-19 팬데믹이 시작되는 동안 젊은이들이 감염될 가능성이 적었는지, 아니면 증상이 발생하여 검사를 받을 가능성이 적었는지는 명확하지 않았습니다.[52] 중국의 후향적 코호트 연구에 따르면 어린이와 성인은 그만큼 감염될 가능성이 높은 것으로 나타났습니다.[53]

보다 철저한 연구 중 2020년 4월 9일의 예비 결과에 따르면 독일의 주요 감염 클러스터의 중심지인 Gangelt에서는 인구 샘플의 15%가 항체 양성 반응을 보였습니다.[54] 뉴욕시의 임산부와 네덜란드의 헌혈자를 대상으로 코로나19 검사를 실시한 결과, 보고된 것보다 더 많은 감염을 나타내는 항체 양성률이 나타났습니다.[55][56] 혈청 유병률 기반 추정치는 일부 연구에서 경미한 증상을 보이는 사람은 검출 가능한 항체를 가지고 있지 않다는 것을 보여주기 때문에 보수적입니다.[57]

2020년 1월 코로나19 기본 재생산지수(R0)의 초기 추정치는 1.4~2.5 사이였지만,[58] 이후 분석에서는 약 5.7(95% 신뢰구간 3.8~8.9)일 수 있다고 주장했습니다.[59]

2021년 12월, 새로운 COVID-19 변종을 포함한 여러 요인으로 인해 사례 수는 계속 증가했습니다. 12월 28일 현재 전 세계에서 282,790,822명이 감염된 것으로 확인되었습니다.[60] 2022년 4월 14일 기준으로 전 세계적으로 5억 건 이상의 사례가 확인되었습니다.[61] 대부분의 사례는 확인되지 않았으며, 건강 측정 평가 연구소는 2022년 초 기준 실제 사례 수를 수십억 건으로 추정했습니다.[62][63]

사망자

2023년 3월 10일 기준 688만[8] 명 이상의 사망자가 COVID-19로 인해 발생했습니다. 첫 사망자는 2020년 1월 9일 우한에서 확인되었습니다.[65] 이러한 수치는 검사량, 의료 시스템 품질, 치료 옵션, 정부 대응,[66] 초기 발병 이후 시간 및 연령, 성별 및 전반적인 건강과 같은 인구 특성에 영향을 받아 지역 및 시간에 따라 다릅니다.[67]

2022년 12월 WHO의 한 연구는 2020년과 2021년 동안 팬데믹으로 인한 초과 사망자를 추정했으며, 다시 ≈ 1,480만 명의 초과 조기 사망자가 발생했다고 결론지었으며, 5월부터 이전 계산을 재확인하고 자세히 설명하고 이를 업데이트하여 비판을 해결했습니다. 이 수치에는 잠재적 생명을 잃은 수년과 같은 조치가 포함되어 있지 않으며 2021년 팬데믹의 주요 사망 원인이 될 수 있습니다.[78][79][64]

증상 발생과 사망 사이의 시간은 6일에서 41일 사이이며, 일반적으로 약 14일 정도입니다.[80] 사망률은 나이에 따라 증가합니다. 사망 위험이 가장 큰 사람은 노인과 기저 질환이 있는 사람입니다.[81][82]

감염 치사율(IFR)은 질병으로 인한 누적 사망자 수를 누적 감염자 수로 나눈 값입니다(무증상 및 진단되지 않은 감염자 포함, 백신 접종된 감염자 제외).[84][85][86] 백분율로 표시됩니다.[87] 다른 연구에서는 이 지표를 감염 치사 위험이라고 합니다.[88][89]

2020년 11월, 네이처의 리뷰 기사는 노인 요양 시설의 사망을 제외한 다양한 국가의 인구 가중 IFR 추정치를 보고했으며 중앙값 범위는 0.24%에서 1.49%[90]로 나타났습니다. IFR은 연령에 따라 증가합니다(10세에 0.002%, 25세에 0.01%에서 55세에 0.4%, 65세에 1.4%, 75세에 4.6%, 85세에 15%). 이 비율은 연령대에 따라 ≈ 10,000배씩 다릅니다. 비교하자면, 중년 성인의 IFR은 연간 치명적인 자동차 사고 위험보다 2배나 높고 계절성 인플루엔자로 사망할 위험보다 훨씬 높습니다.[83]

2020년 12월, 체계적인 검토와 메타 분석에 따르면 인구 가중치 IFR은 일부 국가(프랑스, 네덜란드, 뉴질랜드, 포르투갈)에서 0.5%~1%, 다른 국가(호주, 영국, 리투아니아, 스페인)에서 1%~2%, 이탈리아에서 약 2.5%로 추정되었습니다. 이 연구는 대부분의 차이가 인구의 연령 구조와 연령별 감염 패턴의 상응하는 차이를 반영한다고 보고했습니다.[83] 이번 유행병의 치사율을 메르스와 같은 이전 유행병과 비교한 리뷰도 있었습니다.[91]

비교하자면 미국의 계절성 독감 감염 사망률은 0.1%로 코로나19보다 13배나 낮습니다.[92]

치명률(CFR)

질병

변종

징후 및 증상

코로나19 증상

코로나19 증상은 경증부터 중증까지 다양합니다.[99][100] 흔한 증상으로는 두통, 후각미각상실, 코막힘콧물, 기침, 근육통, 인후통, 발열, 설사, 호흡곤란 등이 있습니다.[99] 같은 감염에 걸린 사람은 증상이 다를 수 있고, 시간이 지나면 증상이 바뀔 수 있습니다. 기침, 가래, 호흡곤란, 발열을 동반한 호흡기 증상 군집 1개, 근육 및 관절통, 두통, 피로감을 동반한 근골격계 증상 군집, 복통, 구토, 설사를 동반한 소화기 증상 군집 등 3개의 공통 증상 군집이 확인되었습니다.[101] 이전에 귀, 코, 목 장애가 없었던 사람들에게 후각 상실과 함께 미각 상실코로나19와 관련이 있으며, 무려 88%의 사례에서 보고되고 있습니다.[102][103][104]

변속기

이 질병은 주로 사람들이 숨을 쉬고, 말하고, 기침, 재채기를 하거나 노래를 부를 때 감염된 사람들이 내뿜는 비말과 작은 공기 입자(에어로졸을 형성하는 입자)를 흡입할 때 호흡기 경로를 통해 전염됩니다.[105][106][107][108] 감염자는 다른 비감염자와 물리적으로 가까울 때 코로나19를 전염시킬 가능성이 더 높습니다. 그러나 감염은 더 먼 거리, 특히 실내에서 발생할 수 있습니다.[105][109]

원인

예방

바이러스 확산 방지를 위한 일반적인 조치

COVID-19 백신은 2019년(COVID-19) 코로나바이러스를 유발하는 바이러스인 중증급성호흡기증후군 코로나바이러스2(SARS-CoV-2)에 대한 후천면역을 제공하기 위한 것입니다. 2023년 3월 현재 197개국 이상에서 55억 명 이상이 하나 이상의 용량[128](총 118억 명)을 투여받았습니다. 옥스퍼드-아스트라제네카 백신이 가장 널리 사용되었습니다.[129] 2022년 6월 연구에 따르면 코로나19 백신은 2020년 12월 8일부터 2021년 12월 8일까지 185개 국가 및 지역에서 1,440만~1,980만 명의 추가 사망을 예방했습니다.[130][131]

2022년 11월 8일 영국 최초의 재조합 단백질 기반 COVID-19 백신(노바백스의 부스터 누백소비드)이 성인용으로 승인되었습니다. 이후 WHO, 미국, 유럽연합, 호주로부터 승인/허가를 받았습니다.[132][133]

WHO는 2022년 11월 12일 글로벌 백신 시장 보고서를 발표했습니다. 보고서는 "부당한 분배는 코로나19 백신만의 것이 아니다"라고 지적하면서 경제적으로 강하지 않은 국가들은 백신을 얻기 위해 고군분투하고 있다고 지적했습니다.[134]

2022년 11월 14일, 중국의 바이오 제약 회사인 캔시노 바이오로직스가 개발한 최초의 흡입 가능한 백신이 중국 상하이시에서 소개되었습니다.[135]

치료

팬데믹의 첫 2년 동안은 구체적이고 효과적인 치료법이나 치료법이 없었습니다.[136][137] 2021년 유럽의약품청(EMA) 인체사용의약품위원회(CHMP)는 경구용 항바이러스 단백질분해효소 억제제팍스로비드(니르마트렐비르+에이즈의약품 리토나비르)를 성인 환자 치료에 승인했습니다.[138] FDA는 나중에 EUA를 제공했습니다.[139]

중증의 경우 지원적 치료에는 증상 완화를 위한 치료, 수액 요법, 산소 지원엎드린 자세, 영향을 받는 다른 중요 기관을 지원하기 위한 약물 또는 장치가 포함됩니다.[143] 더 심한 경우에는 병원에서 치료가 필요할 수 있습니다. 산소 농도가 낮은 분들은 사망률을 줄이기 위해 글루코코르티코이드 덱사메타손을 사용하는 것이 좋습니다.[144] 호흡을 지원하기 위해서는 비침습적 인공호흡과 궁극적으로 기계적 인공호흡을 위한 중환자실 입원이 필요할 수 있습니다.[145] 호흡 부전 문제를 해결하기 위해 체외막 산소화(ECMO)가 사용되었습니다.[146][147]

하이드록시클로로퀸, 로피나비르/리토나비르, 이버멕틴 등 기존 의약품은 유용한 효과가 있다는 좋은 증거가 없어 미국이나 유럽 보건당국의 권고를 받지 않고 있습니다.[136][148][149] 항바이러스제 렘데시비르는 미국, 캐나다, 호주 및 기타 여러 국가에서 다양한 제한 조건으로 사용할 수 있지만 기계적 인공호흡과 함께 사용하는 것이 권장되지 않으며 세계보건기구(WHO)[150]에서 그 효능에 대한 증거가 제한적이기 때문에 전면적으로 권장하지 않습니다.[136]

예후

코로나19의 심각성은 다양합니다. 일반 감기와 같은 다른 흔한 상기도 질환과 유사하게 증상이 거의 없거나 경미한 경과를 밟을 수 있습니다. 3~4%(65세 이상은 7.4%)의 증상이 입원을 유발할 정도로 심합니다.[151] 경증의 경우 일반적으로 2주 이내에 회복되는 반면, 중증 또는 중증 질환이 있는 경우 회복하는 데 3~6주가 걸릴 수 있습니다. 사망자 중 증상 발현부터 사망까지 걸리는 시간은 2주에서 8주 사이입니다. 병원 입원 시 프로트롬빈 시간이 길어지고 C-반응성 단백질 수치가 높아지는 것은 COVID-19의 중증 경과 및 중환자실(ICU)로의 이송과 관련이 있습니다.[152][153]

COVID-19 환자의 5%에서 50% 사이에서 장기간 COVID가 발생하며,[154] 이는 질병의 일반적인 회복 기간 이후에도 장기적인 결과가 지속되는 것을 특징으로 합니다.[155][156] 가장 일반적으로 보고되는 임상 증상은 피로와 기억력 문제뿐만 아니라 권태감, 두통, 호흡 곤란, 후각 상실, 근육 약화, 미열 및 인지 기능 장애입니다.[157][158][159][160]

전략들

완화 목표에는 의료 서비스에 대한 최고 부담(곡선 평탄화)을 지연 및 감소시키고 전체 사례 및 건강 영향을 줄이는 것이 포함됩니다.[161][162] 또한 병상 수, 인력 및 장비 증가와 같은 의료 역량의 점진적인 증가(라인 상향 조정)는 증가하는 수요를 충족하는 데 도움이 됩니다.[163]

의약외품 개입

전 국민 격리, 엄격한 여행 금지 등 보다 과감한 조치가 여러 지역에서 시도되고 있습니다.[173] 중국호주 정부의 접근 방식에는 많은 봉쇄가 포함되어 있으며 가장 엄격한 것으로 널리 간주됩니다. 뉴질랜드 정부의 대응에는 가장 심각한 여행 제한 조치가 포함됐습니다. 한국은 K-방역 프로그램의 일환으로 집단 검진과 현지화된 방역을 도입하고 감염자의 동선에 대한 경보를 발령했습니다. 싱가포르 정부의 대응에는 이른바 '서킷 브레이커 봉쇄'와 피해자에 대한 재정 지원이 포함됐으며, 격리를 위반한 사람들에 대해서도 거액의 벌금을 부과했습니다.[174]

연락처 추적

접촉 추적은 새로 감염된 개인의 최근 접촉자를 식별하고 감염 여부를 검사하려고 시도합니다. 전통적인 접근 방식은 감염자에게 접촉자 목록을 요청한 다음 전화를 걸거나 접촉자를 방문하는 것입니다.[175] 접촉자 추적은 2014년 서아프리카 에볼라 바이러스 유행 당시 널리 사용되었습니다.[176]

또 다른 방법은 모바일 장치에서 위치 데이터를 수집하여 감염자와 상당한 접촉을 한 사람을 식별하는 것이며, 이로 인해 개인 정보 보호 문제가 발생했습니다.[177] 2020년 4월 10일, 구글과 애플은 개인 정보 보호 연락처 추적을 위한 이니셔티브를 발표했습니다.[178][179] 팔란티어 테크놀로지스는 유럽과 미국에서 초기에 코로나19 추적 서비스를 제공했습니다.[180]

건강관리

WHO는 용량 증가와 의료 적응을 근본적인 완화로 설명했습니다.[181] ECDC와 WHO의 유럽 지역 사무소는 실험실 서비스를 시험, 선택적 시술 취소, 환자 분리 및 격리를 포함한 여러 수준의 자원 이동을 위한 병원 및 1차 의료 서비스에 대한 지침을 발표했습니다. 그리고 인력을 양성하고 인공호흡기와 침대를 늘려 집중 치료 능력을 높입니다.[181][182] 팬데믹은 원격 의료의 광범위한 채택을 주도했습니다.[183]

2019

발병은 2019년 11월 우한에서 발견되었습니다. 발견되기 전에는 사람 간 전염이 일어나고 있었을 가능성이 있습니다.[32][33] 2019년 12월부터 시작된 후향적 분석에 따르면 후베이성의 환자 수는 점차 증가하여 12월 20일까지 60명, 12월 31일까지 최소 266명에 달했습니다.[190]

1월 11일, WHO는 중국 국가위생건강위원회로부터 발병이 시장에서의 노출과 관련이 있으며, 중국이 1월 7일 분리한 새로운 유형의 코로나바이러스를 확인했다고 통보받았습니다.[200]

처음에는 약 7일 반에 한 번 꼴로 건수가 두 배로 늘었습니다.[203] 1월 초와 중순, 중국 춘제 이주에 힘입어 다른 중국 지방으로 바이러스가 퍼졌습니다. 우한은 교통의 중심지이자 주요 철도 교류지였습니다.[204] 1월 10일, 바이러스의 유전체가 공개적으로 공유되었습니다.[205] 3월에 발표된 후향적 연구에 따르면 1월 20일까지 6,174명이 증상을 보고했습니다.[206] 1월 24일 보고서에 따르면 인체 전염이 일어날 가능성이 있으며, 의료 종사자들에게 개인 보호 장비를 권장했습니다. 또한 발병의 "팬데믹 잠재력"을 감안할 때 테스트를 권장했습니다.[40][207] 1월 31일, 첫 번째로 발표된 모델링 연구는 피할 수 없는 "세계 주요 도시에서의 독립적인 자생적 발병"을 경고하고 "대규모 공중 보건 개입"을 촉구했습니다.[208]

11월 9일 화이자는 후보 백신에 대한 시험 결과를 발표하여 90%의 감염 예방 효과를 보였습니다. 노바백스는 이날 자사 백신에 대한 FDA 패스트트랙(Fast Track) 신청서를 제출했습니다.[220][221]

12월 14일, 영국 공중보건국은 영국 남동부, 주로 켄트에서 변종이 발견되었다고 보고했습니다. 나중에 알파로 명명된 이 변종은 바이러스를 더 전염성 있게 만들 수 있는 스파이크 단백질의 변화를 보여주었습니다. 12월 13일 현재 영국에서 확인된 감염자는 1,108명입니다.[222][223]

2020년 2월 4일, 미국 보건복지부 장관 알렉스 아자르는 "유의적 위법 행위"와 관련된 경우를 제외한 모든 경우에 백신 제조업체에 대한 책임을 면제했습니다.[224][225]

2021
