COVID-19 drug repurposing research

약물 재배치(drug repositioning)는 승인된 약물을 원래 개발된 것과 다른 질병 또는 질병의 치료를 위해 용도 변경하는 것입니다.[1] 이것은 안전하고 효과적인 COVID-19 치료법을 개발하기 위해 추진 중인 과학 연구의 한 분야입니다.[2][3][4] 다른 연구 방향으로는 코로나19 백신[5] 개발, 회복기 혈장 수혈 등이 있습니다.[6]

기존에 중증급성호흡기증후군(SARS), 중동호흡기증후군(메르스), HIV/AIDS, 말라리아 치료제로 개발됐거나 사용됐던 기존 항바이러스제들이 코로나19 치료제로 연구됐고, 일부는 임상시험에 들어갔습니다.[7][8][9]

2020년 2월 Nature Biotechnology지에 발표한 성명에서, 미국 국립보건원의 Vincent Munster 원장은 "메르스, 사스 및 [SARS-CoV-2] 바이러스의 일반적인 유전체 배치와 일반적인 복제 역학 및 생물학은 매우 유사합니다. 따라서 이러한 코로나바이러스의 비교적 일반적인 부분을 표적으로 하는 약물을 테스트하는 것은 논리적인 단계입니다."[2]

배경

COVID-19와 같은 새로운 신종 감염의 발병은 약물 발견에 사용할 수 있는 시간이 적기 때문에 임상 사용에 적합한 치료법을 발견해야 하는 독특한 문제를 제기합니다.[10] 코로나19 신약을 개발하고 허가하는 과정에서 특히 오랜 시간 지연이 예상됐기 때문에 연구진은 그동안 비용 효율적인 전략으로 승인된 항바이러스제와 다른 약물의 기존 개요를 조사해 왔습니다.[3][10] 2020년 초 수백 개의 병원과 대학이 COVID-19에 대한 용도 변경 가능성이 있는 기존의 안전한 약물에 대한 자체 시험을 시작했습니다.[11]

약물 용도 변경은 일반적으로 개발 파이프라인을 통해 약물을 복용하기 전에 올바른 약물에 대한 인식, 임상 모델에서 약물 효과에 대한 체계적인 평가, 임상 2상에서 유용성 추정의 세 단계가 필요합니다.[12]

위치 변경에 사용되는 한 가지 접근법은 RNA 유전체와 같은 바이러스 관련 표적(즉, 렘데시비르)을 통해 작용하는 약물을 찾는 것입니다. 또 다른 접근법은 폴리펩티드 패킹(즉, 로피나비르)을 통해 작용하는 약물에 관한 것입니다.[10]

팬데믹에 대한 논문을 서둘러 발표하면서 부정확한 과학 출판물의 스캔들이 발생했습니다.[13] 하이드록시클로로퀸과 렘데시비르의 효능을 보고한 일부 초기 연구는 식품의약국(FDA)과 유럽의약품청 등 의약품 기관이 코로나19 과정에 영향을 미치지 않는다는 새로운 증거로 나중에 취소된 긴급사용승인서를 발급하여 허가 외 사용을 승인하도록 설득했습니다.[14] 이러한 잘못된 양성 결과는 기준금리 오류의 관점에서 설명될 수 있으며, 만약 용도 변경 후보자에 대한 기계적 증거와 반대하는 기계적 증거를 신중하게[15] 평가하고 메타 분석과 같은 표준 증거 병합 도구를 일상적으로 사용했다면 COVID-19 치료에 대한 임상 지침의 급격한 변화를 피할 수 있었을 것입니다. 적용된[16]

단클론항체

용도 변경을 위해 조사 중인 단일 클론 항체에는 항 IL-6 제제(tocilizumab)[17] 및 항 IL-8(BMS-986253)이 포함됩니다.[18] (이는 코로나19 전용으로 개발된 신규 단일클론항체 의약품과 병행합니다.)

Mavrilimumab은 인간 과립구 대식세포 집락 자극 인자 수용체(GM-CSF-R)를 억제하는 인간 단일클론항체입니다.[19][20] 코로나19 폐렴과 전신성 과염증 환자의 예후를 향상시킬 수 있는지 연구되었습니다. 한 소규모 연구는 그렇지 않은 사람들에 비해 마브릴리무맙을 사용한 치료의 유익한 효과를 나타냈습니다.[21]

2021년 1월 영국 국민보건국은 면역조절제인 토실리주맙과 사리루맙중환자실에 입원한 코로나19 환자에게 신속하게 투여했을 때 효과가 있다는 지침을 발표했는데, 이는 사망 위험이 24%[22] 감소한다는 연구 결과에 따른 것입니다.

토실리주맙


Actemra 등의 브랜드로 판매되는 Tocilizumab면역억제제류마티스 관절염, 전신성 청소년 특발성 관절염, 소아의 중증 관절염, 코로나19 치료에 사용됩니다. 인터루킨-6 수용체(IL-6R)에 대한 인간화된 단일클론항체입니다. 인터루킨 6(IL-6)은 면역 반응에 중요한 역할을 하는 사이토카인으로 자가면역질환, 다발성골수종전립선암과 같은 많은 질병의 발병기전에 관여합니다. 토실리주맙은 오사카 대학츄가이가 공동 개발했으며 2003년 호프만-라 로슈가 허가했습니다.[23]

Tocilizumab은 2010년 1월 미국에서 의료용으로 승인되었습니다.[24][25]

항응고제

혈액 응고를 방지하는 약물이 치료를 위해 제안되었으며, 저분자 헤파린을 사용한 항응고 치료는 응고병증(상승된 D-dimer)의 징후를 보이는 중증 COVID ‐19의 더 나은 결과와 관련이 있는 것으로 보입니다. 이탈리아에서 여러 항응고제가 테스트되었는데, 저분자량 헤파린이 환자 치료에 널리 사용되어 이탈리아 의약품청이 그 사용에 대한 지침을 발표하도록 하였습니다.[27][28]

과학자들은 헤파린이 SARS-CoV-2 바이러스의 스파이크 단백질과 결합하여 이를 중화시키는 능력을 확인하고 이 약물을 가능한 항바이러스제로 제안했습니다.[29]

예방 및 치료 용량에서 에녹사파린 나트륨의 사용을 연구하는 300명의 환자를 대상으로 한 멀티센터 연구가 4월 14일 이탈리아에서 발표되었습니다.[30]

코로나19 치료제로 항응고제 디피리다몰이 제안돼 [31]임상시험이 진행 중입니다.[32]

항우울제

많은 항우울제들이 항염증 특성을 가지고 있습니다. 파리 지역 병원을 대상으로 한 관찰 연구에서는 이미 항우울제를 복용하고 있던 이 병원에 입원한 코로나19 환자들이 삽관이나 사망 위험을 44%나 줄인 것으로 나타났습니다.[33][34] 플루복사민과 플루옥세틴이 제2형 폐 폐 폐포 세포를 보호함으로써 코로나19 중증 호흡기 증상의 발병을 예방하는 데 기여하고 있는 잠재적 메커니즘은 2022년 3월 리뷰에서 요약되었습니다.[35]

플루복사민

2021년 10월 브라질의 대형 임상시험인 Together trial은 코로나19 조기 진단을 받은 고위험군 외래 환자를 100 mg 플루복사민으로 10일간 매일 2회씩 치료하면 입원 위험이 최대 65% 감소한다고 보고했습니다. 그 효과는 낮은 순응도로 약 32%로 감소했는데, 아마도 불내증 때문일 것입니다. 또한 높은 순응도로 사망자 수가 최대 약 90% 감소했습니다. 이 약은 항염증 효과 때문에 연구되었지만 코로나19에 대한 작용 메커니즘은 여전히 불확실합니다.[36][37][38]

12월 16일, NIH는 플루복사민의 사용이 코로나 관련 입원의 발병률에 영향을 미치지 않는다는 것을 발견했고, 약물에 대한 찬성 또는 반대를 권고하기에 불충분한 증거로 간주했습니다.[39]

12월 23일, 매우 낮은 확실성 증거 하에 온타리오 임상 실무 지침은 증상 발생 후 7일 이내에 경증 환자를 치료하기 위해 약물을 고려할 것을 제안했습니다.[40]

2022년 5월, 미국 식품의약국(FDA)은 이용 가능한 과학적 증거에 대한 검토를 바탕으로 코로나19 치료를 위해 플루복사민에 대한 긴급사용승인(EUA)을 발급해 달라는 요청을 거부했습니다. 코로나19로 입원하지 않은 사람을 치료해 중증질환이나 입원을 예방하는 데 효과가 있을 수 있다는 결론을 내리기에는 자료가 충분하지 않다고 했습니다. EUA 신청서를 제출한 미네소타 대학 데이비드 불웨어 교수는 플루복사민에 대해 보유하고 있는 기준이 팍스로비드와 몰누피라비르, 단클론 다른 대형 제약사 시험과는 다른 기준이라고 말했습니다.[41][42]

역기생충

항바이러스 치료를 위한 숙주 지정 약물의 용도 변경에 대한 아이디어는 르네상스를 경험했습니다.[44] 어떤 경우에는 이 연구가 급성 RNA 바이러스 감염 치료에 사용되는 근본적인 한계를 강조했습니다.[45] 조사된 항기생충제는 클로로퀸,[46] 하이드록시클로로퀸,[47] 메플로퀸,[48][49] 이버멕틴,[50] 아토바퀸 등이 있습니다.[51]

클로로퀸과 하이드록시클로로퀸

하이드록시클로로퀸이 코로나19로 인한 질병이나 사망을 막지 못한다는 세계보건기구(WHO)의 통계.

클로로퀸하이드록시클로로퀸은 일부 자가면역질환에도 사용되는 항말라리아 약물입니다.[52] 클로로퀸은 하이드록시클로로퀸과 함께 코로나19 초기 실험 치료제였습니다.[53] 두 약물 모두 SARS-CoV-2 감염을 예방하지 못합니다.[54][55][56][57][58]

세포 내 바이러스 진입에 필요한 SARS-CoV-2 S2 스파이크 단백질의 절단은 세포막에 위치한 단백질분해효소 TMPRSS2에 의해 수행되거나 엔도리소좀카텝신(주로 카텝신 L)에 의해 수행될 수 있습니다.[59] 하이드록시클로로퀸은 엔도리소좀에서 카텝신 L의 작용을 억제하지만, 카텝신 L 절단은 TMPRSS2 절단에 비해 미미하기 때문에 하이드록시클로로퀸은 SARS-CoV-2 감염을 거의 억제하지 못합니다.[59]

임상시험을 통해 공식적으로 승인되지는 않았지만 코로나19에 입원한 사람들의 치료를 위해 클로로퀸이나 하이드록시클로로퀸을 처음 사용한 국가도 있었습니다(2020년 3월 기준).[60][61] 2020년 4월부터 6월까지 미국에서 사용에 대한 긴급 사용 승인이 있었고 [62]질병의 잠재적 치료를 위해 라벨에서 제외되었습니다.[63] 2020년 4월 24일, FDA는 "심각한 심장 리듬 문제"의 위험을 이유로 COVID-19에 이 약물을 사용하지 말라는 경고를 게시했습니다.[64]

이들의 사용은 국제연대재판과 영국 회복재판에서 코로나19 중증 입원 환자에게 아무런 이득이 없다는 것이 입증되자 코로나19 감염 가능성이 있는 치료제로 철회됐습니다.[65][66] 2020년 6월 15일, FDA는 이 약이 COVID-19에 효과적이거나 그 이점이 "알려지고 잠재적인 위험"을 능가한다고 말하면서 긴급 사용 승인을 취소했습니다.[67][68][69] 2020년 가을, 국립보건원임상시험의 일부를 제외하고 코로나19에 하이드록시클로로퀸을 사용하지 말 것을 권고하는 치료 지침을 발표했습니다.[52]

2021년, 하이드록시클로로퀸은 인도에서 경증 환자에 대한 권장 치료법의 일부였습니다.[70]

2020년 코로나19에 대한 하이드록시클로로퀸의 투기적 사용은 확립된 적응증(말라리아 및 자가면역 질환)을 가진 사람들에게 그것의 가용성을 위협했습니다.[56]

Ivermectin

이버멕틴의 볼앤스틱 모형

이버멕틴은 동물과 사람에게 사용하기에 잘 알려진 항기생충제입니다.[71] 세계보건기구(WHO),[72] 유럽의약품청(EMA),[73] 미국 식품의약국(FDA),[74] 미국 감염병학회(IDSA)[75] 등은 코로나19 치료나 예방을 위해 이버멕틴을 사용하지 말 것을 권고하고 있습니다.

COVID-19 대유행 초기에 실험실 연구는 이버멕틴이 COVID-19를 예방하거나 치료하는 역할을 할 수 있다고 제안했습니다.[76] 온라인 잘못된 정보 캠페인과 옹호는 대중 사이에서 약물의 인지도를 높였습니다. 과학자들과 의사들은 대체로 회의적인 입장을 견지했지만, 일부 국가들은 전염병 통제 노력의 일환으로 이버멕틴을 채택했습니다. 처방전 없이 이버멕틴을 사용하기 위해 필사적으로 사용하는 일부 사람들은 수의학적 제제를 복용했고, 이로 인해 동물 치료를 위한 이버멕틴 공급이 부족하게 되었습니다. FDA는 이 문제에 대해 관심을 끌기 위해 트윗을 통해 "당신은 말이 아닙니다"라고 답했고, 이후 이 문제로 소송을 당했습니다.[77][78]

후속 연구에서는 코로나19에 대한 이버멕틴의 유용성을 확인하지 못했고,[79][80] 2021년에는 이점을 입증하는 많은 연구가 결함이 있거나 오해를 불러일으키거나 사기라는 사실이 나타났습니다.[81][82] 그럼에도 불구하고, 이버멕틴에 대한 잘못된 정보는 소셜 미디어에서 계속 전파되었고 이 약은 백신 접종 반대론자들과 음모 이론가들에게 셀레브레의 원인으로 남아있었습니다.[83]

항바이러스제

MERS-CoV, SARS-CoV, 웨스트 나일 바이러스 등 기존에 다른 바이러스에 대항하여 개발된 승인된 항바이러스제의 용도 변경에 연구가 집중되고 있습니다.[84] 여기에는 파비피라비르,[84] 렘데시비르, [85][86]리바비린,[87] 트리아자비린우미페노비르가 포함됩니다.[88]

Hela 세포를 이용한 SARS-CoV-2 in vitro test에 아르테수네이트/피로나리딘의 조합이 억제 효과가 있는 것으로 밝혀졌습니다. 아르테수네이트/피로나리딘은 24시간 후 99% 이상의 바이러스 역가 억제율을 보였고, 세포 독성도 감소했습니다.[89] 2020년 7월에 발표된 사전 인쇄물은 Pyronariidine과 아르테수네이트가 인간 폐 상피세포(Calu-3)를 이용하여 SARS-CoV-2 및 인플루엔자 바이러스에 대해 항바이러스 활성을 나타낸다고 보고했습니다.[90] 한국[91][92][93] 남아프리카에서 임상 2상을 진행 중입니다.[94]

몰누피라비르는 인플루엔자를 치료하기 위해 개발된 약입니다. 코로나19 치료제로 3상 시험 중입니다.[95][96][97][98][99] 2020년 12월, 과학자들은 인플루엔자 치료를 위해 개발된 항바이러스제 몰누피라비르가 인간 젊은 성인 인구에서 확산된 사스-CoV-2와 코로나19 전염이 밀접하게 유사한 것으로 밝혀진 흰 족제비에서 24시간 이내에 사스-CoV-2 전염을 완전히 억제할 수 있다고 보고했습니다.[100][101] 2021년 10월 1일 현재 동료 심사를 받지 않은 임상 시험에 따르면, 경구 복용한 몰누피라비르는 코로나19 진단을 받은 환자의 입원 위험을 줄이고 사망을 예방할 수 있습니다. 약이 효과적이기 위해서는 조기에 투여되어야 합니다.[102][103] 2022년 1월 1일 현재 몰누피라비르는 영국, 인도, 미국에서 COVID-19에 대한 긴급사용 승인을 받았습니다.[104]

Niclosamide는 한국에서 수행된 시험관 내 약물 스크리닝 분석에서 후보 항바이러스제로 확인되었습니다.[105]

CLpro-1, GC376, Rupintrivir 등의 실험실에서 프로테아제 3CLpro를 표적으로 하는 프로테아제 저해제가 연구 개발되고 있습니다.[106][107][108]

코로나바이러스 종은 리바비린에 대한 고유한 내성을 가지고 있습니다.[109]

Sofosbuvir/daclatasvir는 C형 간염을 치료하기 위해 개발된 약물 조합입니다. 2020년 10월, 메타 분석에 따르면 입원 환자에게 투여했을 때 약물 조합으로 모든 원인의 사망 위험이 현저히 낮은 것으로 나타났습니다.[110]

Favipiravir

파비피라비르는 일본에서 인플루엔자 치료에 승인된 항바이러스제입니다.[111][84] 다른 항바이러스제에 비해 파비피라비르가 코로나19 환자의 결과를 개선할 수 있다는 증거는 제한적이지만 결론을 내리기 전에 더 엄격한 연구가 필요합니다.[112]

우한선전의 중국 임상시험에서 파비피라비르가 "명백한 효과"를 보였다고 주장했습니다.[113] 선전시 환자 35명 가운데 중간값 4일 만에 음성 판정을 받았고, 받지 않은 환자 45명 가운데 질병 기간은 11일이었습니다.[114] 우한에서 폐렴 환자 240명을 대상으로 실시한 연구에서 절반은 파비피라비르, 절반은 우미페노비르를 투여받았습니다. 연구원들은 환자들이 파비피라비르로 치료했을 때 기침과 열에서 회복되는 속도가 더 빠르다는 것을 발견했지만, 인공호흡기로 치료해야 하는 질병의 더 진행 단계에 각 그룹의 환자 수에는 변화가 없다는 것을 발견했습니다.[115]

2020년 3월 22일, 이탈리아는 COVID-19에 대한 실험적 사용을 승인하고 질병의 영향을 가장 많이 받는 3개 지역에서 시험을 진행하기 시작했습니다.[116] 이탈리아 제약청은 국민들에게 이 약을 지지하는 기존의 증거가 부족하고 예비적이라고 상기시켰습니다.[117]

2020년 5월 30일, 러시아 보건부는 아비파비르(Avifavir)라는 이름의 파비피라비르의 제네릭 버전을 승인했으며, 이는 임상 1상에서 매우 효과적인 것으로 입증되었습니다.[118][119][120]

2020년 6월, 인도는 글렌마크 제약이 개발한 파비플루(FabiFlu)라는 파비파비르의 일반 버전을 코로나19 경증에서 중등증 환자의 치료에 사용하는 것을 승인했습니다.[121]

2021년 5월 26일, 체계적인 검토 결과 입원 후 첫 7일 동안 투여했을 때 임상적 개선 가능성이 24% 더 높았지만, 입원 환자 및 경증 또는 중등증 환자를 포함한 그룹의 사망률은 통계적으로 유의하게 감소하지 않았습니다.[122][123]

Lopinavir/ritonavir

SARS-CoV-2의 게놈: 회색 쐐기는 주요 코로나바이러스 단백질분해효소인 3CLpro가 다단백질을 절단하는 곳을 보여줍니다.

2020년 3월, SARS-CoV-2 바이러스의 주요 단백질분해효소(3CLpro)가 감염 후 약물의 표적으로 확인되었습니다. 효소복제와 관련된 다단백질을 처리하는 데 필수적입니다. 이 효소를 찾기 위해 과학자들은 2020년 1월 중국 연구진이 발표한 유전체를 이용해 주요 단백질분해효소를 분리했습니다.[124] 로피나비르와 리토나비르와 같은 인간 면역결핍 바이러스(HIV)를 치료하기 위해 승인된 단백질분해효소 억제제코로나바이러스, 사스 및 메르스에 대한 활성의 예비 증거를 가지고 있습니다.[7][125] 이들은 잠재적인 병용 요법으로 2020년 코로나19 글로벌 연대 프로젝트의 3상 두 개의 암에 함께 사용됩니다.[125][126] 로피나비르와 리토나비르를 병용한 중국의 예비 연구에서는 코로나19로 입원한 사람들에게서는 효과가 없는 것으로 나타났습니다.[127]

항바이러스제 로피나비르리토나비르의 조합인 로피나비르/리토나비르(칼레트라)에 대한 한 연구는 "이익이 관찰되지 않았다"는 결론을 내렸습니다.[127][128] 이 약물은 HIV가 단백질분해효소에 결합하여 복제하는 것을 억제하도록 설계되었습니다. 콜로라도 대학의 한 연구팀은 사스-CoV-2의 단백질분해효소와 결합할 화합물을 찾기 위해 약물을 변형시키려고 노력하고 있습니다.[129] 과학계 내에서는 HIV/AIDS를 위해 특별히 개발된 약을 그들이 목표로 하는 특정 HIV-1 단백질분해효소가 없는 바이러스에 효과적일 가능성이 없기 때문에 자원을 다른 용도로 사용하도록 지시하는 것에 대한 비판이 있습니다.[2] WHO는 로피나비르/리토나비르를 국제연대 재판에 포함시켰습니다.[130]

6월 29일, 영국 회복 시험의 수석 조사관들은 치료 기간 28일 동안 심각한 COVID-19 감염으로 입원한 1,596명을 대상으로 로피나비르-리토나비르를 사용한 임상적 이점이 없다고 보고했습니다.[131][132]

2020년 10월에 발표된 연구 결과, SARS-CoV-2 스파이크(S) 단백질을 표적으로 하는 미국 식품의약국(FDA)의 승인을 받은 약물을 스크리닝하는 것은 현재의 불균형한 로피나비르 조합 공식이 수용체 결합 도메인-인간 안지오텐신 전환 효소-2(RBD-hACE2) 상호작용에 대한 리토나비르의 차단 활성을 실제로 방해할 수 있다고 제안했습니다. 따라서 COVID-19 사례에서 치료 효과를 효과적으로 제한합니다.[133]

2022년 파노라마 시험리토나비르와 결합된 니르마트렐비르와 몰누피라비르의 효과를 테스트하고 있으며, 50세 이상 및 기저 건강 상태로 인해 더 높은 위험에 처한 사람들의 입원을 예방하고 더 빠른 회복을 돕습니다.[134][135] 2022년 3월 현재 16,000명 이상이 참가자로 등록되어 COVID-19 항바이러스제에 대한 가장 큰 연구입니다.[136]

렘데시비르


베클루리라는 브랜드로 판매되는 [137][138]렘데시비르바이오의약품 회사 길리어드 사이언스가 개발한 광범위 항바이러스제입니다.[139] 정맥 주사를 통해 투여됩니다.[140][141] COVID-19 팬데믹 기간 동안, 렘데시비르는 수많은 국가에서 COVID-19 치료를 위한 긴급 사용을 승인받거나 승인받았습니다.[142]

렘데시비르는 원래 C형 간염을 치료하기 위해 개발되었으며, [143]이후 에볼라 바이러스 질병과 마르부르크 바이러스 감염[144] 여부를 조사한 후 코로나19의 감염 후 치료제로 연구되었습니다.[145]

렘데시비르는 GS-441524 일인산의 세포 내 전달 및 바이러스 RNA 중합효소리보뉴클레오티드 유사체 억제제인 GS-441524 삼인산으로의 생체 내 변형을 허용하기 위한 전구약물입니다.[146]

건강한 자원봉사자들에게 가장 흔한 부작용은 효소의 혈중 농도 상승입니다.[137] 코로나19에 걸린 사람들에게 가장 흔한 부작용은 메스꺼움입니다.[137] 부작용으로는 간 염증과 메스꺼움, 저혈압, 땀 흘림과 함께 주입과 관련된 반응이 있을 수 있습니다.[147]

미국 식품의약국(FDA)은 그것을 일류 의약품으로 간주합니다.[148]

면역조절치료

바리티치닙

2022년 5월 미국 식품의약국(FDA)은 보충산소, 비침습적 또는 침습적 기계적 인공호흡 또는 체외막산소화(ECMO)가 필요한 입원 성인의 코로나19 치료를 위한 바릭티닙을 승인했습니다.[149][150] 바릭티닙은 FDA 승인을 받은 최초의 코로나19 면역조절 치료제입니다.[150]

미국에서 barictinib은 보충 산소, 비침습적 또는 침습적 기계 환기 또는 체외막 산소화가 필요한 2세에서 18세 미만의 입원 환자에서 코로나19 치료를 위한 긴급사용승인(EUA)에 따라 승인됩니다.[149]

부데소니드

시클레소니드

덱사메타손

주사용 덱사메타손 바이알

덱사메타손류마티스 질환, 피부 질환, 천식만성 폐쇄성 질환과 같은 여러 질환에 사용되는 코르티코스테로이드 약물입니다.[161] 2020년 2월에 발표된 급성 호흡곤란 증후군(ARDS) 치료에서 덱사메타손에 대한 다중 중심 무작위 대조 시험은 기계적 인공호흡 및 사망률 감소를 보여주었습니다.[162] 덱사메타손은 산소 보충이 필요한 사람에게만 도움이 됩니다. WHO는 7개의 무작위 시험을 분석한 결과,[163] 중증 또는 위중한 질병을 가진 사람들의 치료 지침에 전신 코르티코스테로이드를 사용하고, 중증 질환 기준에 맞지 않는 사람들에게는 사용하지 말 것을 권고하고 있습니다.[164]

미국감염학회(IDSA) 가이드라인 패널은 중증 코로나19 환자에게 글루코코르티코이드를 사용할 것을 제안합니다. 여기서 중증은 실내 공기에서 산소 포화도(SpO2)가 94% 이하인 환자와 보충 산소, 기계적 환기 또는 체외막 산소화(ECMO)가 필요한 환자로 정의됩니다.[172] IDSA는 보충 산소가 필요한 저산소혈증이 없는 COVID-19 환자에게 글루코코르티코이드의 사용을 금지할 것을 권고합니다.[172]

2020년 9월, 유럽의약품청(EMA)은 보충 산소 치료가 필요한 성인과 청소년(12세부터 체중이 최소 40kg(88파운드))에게 덱사메타손의 사용을 승인했습니다.[176] 덱사메타손은 입으로 복용하거나 정맥에 주사 또는 수액(드립)으로 투여할 수 있습니다.[176]

히드로코르티손

2020년 9월 WHO 코로나19 치료제 신속증거평가(REACT) 워킹그룹이 발표한 메타분석에서 히드로코르티손이 다른 일반 치료제나 위약에 비해 위독한 코로나19 환자의 사망률을 낮추는 데 효과적인 것으로 나타났습니다.[177]

코르티코스테로이드의 사용은 스트롱길로이드증을 가진 사람들에게 심각하고 치명적인 "과감염" 증후군을 일으킬 수 있는데, 이것은 Strongyloides stercoralis 기생충에 노출된 집단에서 근본적인 질환일 수 있습니다. 스테로이드 치료 전에 이버멕틴을 추정적으로 사용함으로써 이러한 위험을 완화시킬 수 있습니다.[178]

메틸프레드니솔론

ICU 입원 예방, 기계적 인공호흡기의 필요성 또는 사망률의 복합적인 종말점의 경우, 입원 초기에 사용했을 때 메틸프레드니솔론의 경우 단일 환자에게 혜택을 주기 위해 치료(NNT)에 필요한 는 5개에 불과했습니다. 메틸프레드니솔론이 사망을 피하기 위해 필요한 NNT는 모든 입원 환자에서 8개에 불과했습니다.[181][182] 이것은 기계적 인공호흡 중인 환자의 경우 NNT가 8이고 환자의 경우 사망을 예방하기 위해 산소가 필요한 환자가 25인 덱사메타손(위의 덱사메타손 참조)에 대한 RECOVER 시험(NCT0423592)과 대조적입니다.

비타민

비타민C

비타민C를 포함한 미량 영양소의 보충은 대사 요구 증가로 인해 감염의 급성기에 혈청 및 백혈구의 비타민C 수치가 고갈됨에 따라 COVID-19의 지원 관리의 일환으로 제안되었습니다.[184] 고용량 정맥내 비타민 C의 사용법이 연구되었습니다.[184] ClinicalTrials.gov 에 따르면 코로나19에 감염된 사람, 입원 환자, 중증 환자를 완료했거나 모집 중인 비타민C를 포함한 50개의 임상시험이 완료되거나 진행 중입니다.

비타민D

경구 비타민 D 정제

코로나19 대유행 기간 동안 중증 코로나19와 비타민D 결핍의 위험인자가 상당 부분 겹친다는 에서 비타민D 상태와 보충제에 대한 관심이 있었습니다.[188] 여기에는 비만, 고령, 흑인 또는 아시아 민족 출신 등이 포함되며, 특히 비타민 D 결핍이 이들 그룹에서 흔히 발생한다는 점이 눈에 띕니다.[188]

특히 서구 인구의 비타민 D 결핍 수준을 고려할 때 비타민 D 보충제 복용을 고려하라는 일반적인 권고가 반복되었습니다.[190] 2021년 2월 현재, 영국 국립 보건 관리 연구소(NICE)는 햇빛에 거의 노출되지 않는 사람들을 위해 소량의 비타민 D 보충제를 계속 권장하고 있지만, 임상 시험의 일부를 제외하고는 실무자들이 코로나19를 예방하거나 치료하기 위해서만 비타민 D 보충제를 제공해서는 안 된다고 권고했습니다.[190]
