출생 간격
Birth spacing출산 간격, 임신 간격, 출산 간 간격(IBI) 또는 출산 간 간격은 임신 전 여성이 임신한 후 얼마 지나지 않아 다시 임신하거나 출산하는 경우를 말한다. 임신과 멀리 떨어져 있는 임신과 관련된 건강 위험이 있지만, 대부분의 건강 위험은 너무 가까이서 발생하는 출산과 관련이 있다.[1] 세계보건기구는 임신 24개월을 권고한다.[2] 임신이 낙태나 유산으로 끝난 경우, 일반적으로 6개월의 짧은 간격이 적절할 수 있다.[3][2] 산모가 이전 제왕절개를 한 적이 있다면 출산 중 산모의 자궁 파열 위험이 있어 출산 전 다시 출산하기 전에 기다리는 것이 바람직하며, 최소 출산 간 간격은 1년에서 3년이다. 5년 이상의 임신 간격은 임신 전 위험 증가와 관련이 있다.[4] 짧은 예비 기간 동안 발생하는 전세계 공중 보건 부담은 상당하다.[2] 가족계획은 준비간격을 증가시키는데 도움을 줄 수 있다.[5]
출생 간격에 영향을 미치는 요인
피임약의 사용과 같은 가족계획은 준비간격을 증가시킬 수 있다.[5] 모유수유와 모유수유 연장도 젖먹이 아메르헤아로 인한 출생 간격을 늘릴 수 있다.[6][7][8][9]
성별과 피임약의 사용,[10] 의료의 가격과 이용 가능성, 빈곤에 대한 문화적 종교적 태도는 모두 임신 간 간격에 영향을 미칠 수 있는 요인이다.[11]
건강 위험
짧은 준비 간 간격
출산 간 짧은 간격은 산모 사망률,[5][12] 사산율, 아동 사망률의 증가와 관련이 있다.[13] 18개월 미만의 예비간격은 임신 전 출산의 위험 증가와 관련이 있다.[3]
이전 제왕절개 후 짧은 예비간격은 이전 제왕절개(VBAC) 이후 질 출산을 억제할 수 있다. 한 연구에서 6개월보다 짧은 예비간격은 자궁파열 위험 증가, 주요 질병 및 하나 이상의 이전 제왕절개를 가진 산모에게서 질 분만 시 수혈 위험 증가와 관련이 있었다.[14]
임신중절 간격이 짧은 합병증은 임신 전기에 비해 낙태나 유산 후 합병증이 낮다.[13]
긴 예비 간 간격
5년 이상의 예비간격은 예전의 위험 증가와 관련이 있다.[4][12]
공중위생
짧은 예비 기간 동안 발생하는 전세계 공중 보건 부담은 상당하다.[2] 개발도상국의 경우 형 이후 2년 이상 출생한 아이는 유아기에 사망 위험이 60% 증가한 반면, 2~3년 사이 출생자는 4~5년 간격으로 출생한 아이에 비해 10% 증가했다.[15] 세계보건기구를[2] 포함한 다양한 기관들은 출생 간격을 공중 보건 개입의 중요한 영역으로 파악했다.
수렵채집사회에서는.
전통적인 수렵채집 사회에서 출생 간격이 더 높은 것으로 밝혀졌다. 뉴기니 고원의 게인즈족의 중위 출생 간격은 3.6년(43개월),[16] 아프리카 !쿵족 출생 간격은 3.7년(44개월)으로 보고되었다.[17]
동물에서
야생 유인원의 출산간격은 고릴라의 경우 3.8년, 침팬지의 경우 5.6년, 오랑우탄의 경우 7.7년으로 사람보다 상당히 긴 것으로 보고되고 있다.[16]
