필로폰은 약물사용장애, 심장질환, 심한 동요나 불안의 이력이 있는 개인이나 현재 동맥경화증, 녹내장, 갑상선항진증, 심한 고혈압 등을 겪고 있는 개인에게 억제된다.[22]식약처는 과거 다른 자극제에 과민반응을 보이거나 현재 모노아민 산화효소 억제제를 복용하고 있는 사람은 필로폰을 복용하면 안 된다고 명시하고 있다.[22]FDA는 또 조울증, 우울증, 혈압 상승, 간 또는 신장 질환, 조증, 정신이상, 레이노 현상, 발작, 갑상선 질환, 틱스, 투렛 증후군을 가진 개인에게 필로폰을 복용하면서 증상을 관찰할 것을 권고하고 있다.[22]FDA는 성장이 저해될 수 있기 때문에 치료 중 성장 중인 어린이와 청소년의 키와 몸무게를 모니터링할 것을 권고하고 있다.[22]

역효과

2010년 한 연구에서는 마약 중독 전문가들의 진술을 토대로 각종 불법 및 합법적 마약의 순위를 매겼다.필로폰은 사회에 4번째로 피해가 큰 것으로 나타났다.[28]

물리적인

필로폰의 물리적 효과로는 식욕 저하, 과잉행동, 동공 팽창, 홍조, 과도한 땀, 운동 증가, 입과 치아 갈기('메이드 입'으로 이어짐), 두통, 불규칙한 심장박동(보통 가속 심장박동 또는 심장박동 지연), 빠른 호흡, 고혈압, 저혈압 등이 있다.요소,[22][29] 높은 체온, 설사, 변비, 흐릿한 시야, 현기증, 경련, 저림, 떨림, 건성피부, 여드름, 창백한 외모.장기간의 필로폰 사용자들은 피부에 염증이 있을 수 있다;[30][31][32] 이것들은 가려움으로[31] 인한 긁힘이나 벌레들이 피부 밑으로 기어간다는 믿음으로 인해 야기될 수 있고,[30] 그 피해는 잘못된 식습관과 위생으로 인해 복합된다.[32]필로폰 과다 투약과 관련된 수많은 사망자도 보고되었다.[33][34]

필로폰 사용자와 중독자는 약칭 필로폰 입으로 알려진 상태에서 투여 경로에 관계없이 비정상적으로 빠르게 치아를 잃을 수 있다.[35]이 상태는 일반적으로 삼켜거나, 담배를 피우거나, 들이마시는 것보다는 약을 주입하는 사용자들에게 가장 심각하다.[35]미국치과협회에 따르면 필로폰 주둥이는 "약물에 의한 심리적, 생리학적 변화가 복합적으로 작용하여 생긴 것으로 보아 구강 위생상태가 좋지 않은 기간이 길어지고 고칼로리, 탄산음료, 브루크시즘(스스로 갈아서 꽉 쥐는 것)[35][36]의 잦은 섭취가 원인일 것"이라고 한다.건입도 다른 자극제의 일반적인 부작용인 만큼 충치가 심한 것으로 알려져 있지 않은 만큼 필로폰 관련 충치가 사용자의 다른 선택으로 인한 것이라는 연구자들의 의견이 많다.이들은 현재 이용자에 대한 고정관념을 새로운 이용자에 대한 억제책으로 만들기 위해 부작용이 과장되고 스타일화되었음을 시사한다.[23]

성병감염

신경독성 및 신경면역학

필로폰은 실험용 동물과 사람 모두의 도파민성 뉴런에 직접 신경독성이 있다.[20][21]흥분성, 산화성 스트레스, 대사 절충, UPS 기능 장애, 단백질 질화, 소포성 망막 스트레스, p53 표현 및 기타 과정이 이 신경독성에 기여했다.[48][49][50]도파민성 신경독성에 걸맞게 필로폰 사용은 파킨슨병 발병 위험이 높은 것과 관련이 있다.[51]도파민성 신경독성 외에도, 인간에 대한 증거를 검토한 결과, 고선량 필로폰 사용이 세로토닌 신경독성이 될 수 있다는 것을 알 수 있었다.[21]높은 코어 온도는 필로폰의 신경독성 효과의 증가와 상관관계가 있다는 것이 입증되었다.[52]피부양자 필로폰의 투약시 배출이 일반 투약기간을 넘어 수개월 이상 지속되는 사후 투약으로 이어질 수 있다.[50]

인간 필로폰 사용자를 대상으로 한 자기공명영상 연구에서도 뇌 구조와 기능에서 신경세포가 생성되거나 신경세포가 비정상적으로 변했다는 증거가 발견됐다.[21]특히 필로폰은 레크리에이션용 필로폰 사용자의 경우 백색질의 초강도와 비대증, 해마의 현저한 위축, 정백피질, 변연피질, 마비피질 등에서 회백질을 감소시키는 것으로 나타난다.[21]또한, N-아세틸라스파테이트크레아틴의 감소와 콜린묘이노시톨의 증가와 같은 레크리에이션 사용자들에게 대사 무결성과 합성성의 바이오마커 수준의 부정적인 변화가 일어난다는 증거가 제시된다.[21]

필로폰은 인간 아스트로사이티에서 TAAR1을 활성화하고 그 결과 cAMP를 발생시키는 것으로 나타났다.[51]아스트로사이테 국소화된 TAAR1의 활성화는 필로폰이 막 결합형 EATC2(SLC1A2) 수준을 감쇠시켜 이들 세포에서 기능을 수행하는 메커니즘으로 보인다.[51]

치료 및 관리

암페타민, 필로폰 또는 코카인 중독에 대한 12가지 심리사회적 개입이 포함된 50가지 재판에 대한 2018년 체계적 검토 및 네트워크 메타분석 결과, 우발적 관리지역사회 강화 접근방식을 모두 갖춘 결합치료의 효능(즉, 금주율)과 수용성(즉, 최저)이 가장 높은 것으로 나타났다.중퇴 비율.[85]분석에서 조사된 다른 치료 방법으로는 우발적 관리 또는 공동체 강화 접근법을 이용한 단일 치료법, 인지 행동 치료법, 12단계 프로그램, 비전형 보상 기반 치료법, 심리역학 치료법 및 이와 관련된 기타 결합 치료법이 있다.[85]

2019년 12월 현재 필로폰 중독에 대한 효과적인 약리 요법은 없다.[86][87][88]2019년부터 체계적 검토와 메타분석이 암페타민 및 필로폰 중독에 대한 RCT에서 사용된 17가지 약리요법의 효능을 평가한 결과 [87]메틸페니데이트가 암페타민이나 필로폰 자가 투여를 줄일 수 있다는 저강도 증거만 발견됐다.[87]There was low- to moderate-strength evidence of no benefit for most of the other medications used in RCTs, which included antidepressants (bupropion, mirtazapine, sertraline), antipsychotics (aripiprazole), anticonvulsants (topiramate, baclofen, gabapentin), naltrexone, varenicline, citicoline, ondansetron, prometa, riluzole, atomoxetine, dextroam피타민, 모다피닐.[87]

의존과 철수

일반 필로폰 사용 시 내성이 생길 것으로 예상되며, 레크리에이션 사용 시 이 내성이 빠르게 발생한다.[89][90]의존적인 사용자의 경우, 탈퇴 증상은 약물 내성 수준과 긍정적인 상관관계가 있다.[91]필로폰 투하로 인한 우울증코카인 투하보다 더 오래 지속되고 더 심하다.[92]

현행 필로폰 레크리에이션 사용자들의 약물 의존성탈퇴에 대한 코크란 리뷰에 따르면 "만성 헤비유저가 갑자기 [메탐페타민] 사용을 중단했을 때 마지막 투약 후 24시간 이내에 발생하는 시간제한 탈퇴 증후군을 보고하는 경우가 많다"[91]고 밝혔다.만성 고선량 사용자에서 탈퇴 증상이 자주 나타나 최대 87.6%의 사례에서 발생하며, 첫 주 동안 현저한 '충돌' 단계가 발생하면서 3~4주 동안 지속된다.[91]필로폰 금단 증상은 불안, 약물 욕구, 불규칙한 기분, 피로, 식욕 증가, 운동 증가 또는 운동 감소, 동기부여 부족, 불면증 또는 졸림, 생생하거나 명쾌한 꿈 등을 포함할 수 있다.[91]

산모의 혈류 속에 존재하는 필로폰은 태반을 거쳐 태아에게 전달돼 모유로 분비될 수 있다.[92]필로폰 투약 산모에게서 태어난 영아는 신생아 금단증후군을 겪을 수 있는데, 수면 패턴이 비정상적이고, 영양 상태가 좋지 않으며, 진동이 심하고, 과음증이 동반되는 증상이 나타난다.[92]이 금단 증후군은 비교적 경미하며 약 4%의 환자에서만 의료 개입이 필요하다.[92]

다른 약과 달리 태아에게 필로폰에 노출된 아기는 즉각적인 금단 증상을 보이지 않는다.대신에, 인지 및 행동 문제는 아이들이 학령기에 이르면 나타나기 시작한다.[93]

330명의 어린이를 대상으로 한 예비 코호트 연구에서는 3세 때 필로폰 피폭 어린이의 정서적 반응성이 증가했으며 불안과 우울증의 징후가 증가했으며, 5세 때는 어린이의 외향성 주의력 결핍/과잉행동 장애 비율이 더 높은 것으로 나타났다.[94]

과다 복용

필로폰 과다 복용으로 광범위한 증상이 나타날 수 있다.[4][22]필로폰을 적당히 과다복용하면 심장 박동, 혼란, 난감하고/또는 고통스러운 배뇨, 고혈압 또는 저혈압, 높은 체온, 과다 및/또는 과민반응 반사작용, 근육통, 심한 동요, 빠른 호흡, 떨림, 요로결핍, 소변 통과 불능 등의 증상이 나타날 수 있다.[4][29]과다복용은 아드레날린 폭풍, 필로폰 정신질환, 소변 출력이 현저히 감소하거나 아예 없거나 심장에 의한 쇼크, 뇌출혈, 순환기 붕괴, 과피레시아(위험할 정도로 높은 체온), 폐고혈압, 신장 기능부전, 급격한 근육파괴, 세로토닌 증후군 등의 증상을 일으킬 수 있다.정형화된 형태("착각적")[sources 1]를 말한다.필로폰 과다 복용으로 도파민성, 세로토닌성 신경독성으로 인한 가벼운 뇌손상이 발생할 가능성도 높다.[98][21]필로폰 중독으로 인한 사망은 일반적으로 경련과 혼수상태가 뒤따른다.[22]

정신병

필로폰을 사용하면 흥분제 정신착란이 일어나 다양한 증상(예: 편집증, 환각증, 망상증, 망상증)[4][99]암페타민, 덱스트로암페타민, 필로폰 사용유발 정신질환 치료에 대한 코크란 콜라보레이션(Cochrane Collaboration) 검토 결과 사용자의 약 5~15%가 완쾌되지 않는 것으로 나타났다.[99][100]같은 리뷰는 적어도 한 번의 실험을 바탕으로 항정신병 약물들이 급성암페타민 정신병의 증상을 효과적으로 해결한다고 주장한다.[99]암페타민 정신병은 또한 치료로 인한 부작용으로서 때때로 발병할 수 있다.[101]

응급처치

급성 필로폰 중독은 주로 증상을 치료하여 관리되며, 초기에는 활성탄 투여와 진정제 투여가 포함될 수 있다.[4]필로폰 중독의 경우 혈액투석이나 복막투석 등에 대한 증거가 부족해 유용성을 판단하지 못하고 있다.[22]강제산 이뇨증(예: 비타민 C)은 필로폰 배설을 증가시키지만 산증을 악화시키거나 발작이나 횡문근융해증을 일으킬 위험이 있으므로 권장하지 않는다.[4]고혈압은 뇌내 출혈(즉, 뇌출혈)의 위험을 나타내며,[4] 심할 경우 정맥내 페놀라민 또는 니트로프루사이드로 치료한다.혈압은 벤조디아제핀으로 충분한 진정제를 투여하고 진정된 환경을 제공하면 점차 낮아지는 경우가 많다.[4]

할로페리돌과 같은 항정신병 약물들은 필로폰 과다복용으로 인한 동요와 정신병을 치료하는데 유용하다.[102][103]지방질 성질과 메토프로롤라베탈롤과 같은 CNS 침투가 있는 베타 차단제는 CNS와 심혈관 독성을 치료하는 데 유용할 수 있다.[104]알파와 베타 차단제 라베탈롤 혼합은 필로폰에 의해 유발되는 동반성 빈맥과 고혈압 치료에 특히 유용하다.[102]필로폰 독성 치료에 베타 차단제를 사용하면서 '무저항 알파 자극' 현상은 보고되지 않고 있다.[102]

상호작용

필로폰은 간 효소 CYP2D6에 의해 대사되므로 CYP2D6 억제제는 필로폰의 소실 반감기를 연장시킨다.[105]필로폰은 모노아민 산화효소 억제제(MAOI)와도 상호작용하는데,[22] MAOI와 필로폰 모두 혈장 카테콜아민을 증가시키기 때문에 두 가지를 동시에 사용하는 것은 위험하다.필로폰은 진정제압박제의 효과를 감소시키고 항우울제와 다른 자극제의 효과를 증가시킬 수 있다.[22]필로폰은 각각 심혈관계와 인지에 미치는 영향 때문에 항고혈압제항정신병 약물들의 효과를 상쇄시킬 수 있다.[22]위장성분과 소변의 pH는 필로폰의 흡수와 배출에 영향을 미친다.[22]구체적으로 산성물질은 필로폰 흡수를 줄이고 소변 배설을 증가시키는 반면 알칼리성 물질은 반대로 작용한다.[22]pH가 흡수에 미치는 영향으로 위산을 줄이는 양성자 펌프 억제제는 필로폰과 상호작용을 하는 것으로 알려져 있다.[22]

약리학

필로폰은 뇌 카테콜아민 시스템을 조절하는 GPCR(단백질결합수용체)인 미량아민 관련 수용체 1(TAAR1)의 잠재적 완전작용제로 확인됐다.[106][107]TAAR1의 활성화는 순환 아데노신 모노인산염(cAMP) 생산을 증가시키고 도파민 트랜스포터(DAT), 노레피네프린 트랜스포터(NET), 세로토닌 트랜스포터(SERT)의 이송 방향을 완전히 억제하거나 역전시킨다.[106][108]필로폰이 TAAR1에 결합하면 단백질 키나아제 A(PKA) 신호와 단백질 키나아제 C(PKC) 신호를 통해 전달인산화를 유발해 궁극적으로 모노아민 전달체내실화 또는 역기능을 초래한다.[106][109]필로폰은 또한 세포내 칼슘을 증가시키는 것으로 알려져 있는데, 이는 Ca2+/calmodulin 의존성 단백질 키나아제(CAMK)의 신호 경로를 통해 DAT 인산화 작용과 연관되어 도파민 유출을 발생시키는 효과가 있다.[110][111][112]TAAR1은 G단백질결합내정 칼륨채널의 직접 활성화를 통해 뉴런의 발화율을 감소시키는 것으로 나타났다.[113][114][115]아스트로사이테스 내 필로폰에 의한 TAAR1 활성화는 글루탐산염 수송기의 일종인 EATH2의 막 표현과 기능을 부정적으로 변형시키는 것으로 보인다.[51]

필로폰도 알파-2 아드레날린 수용체시그마 수용체작용제σ보다2 σ1 친화력이 뛰어나 모노아민 산화효소 A(MAO-A)와 모노아민 산화효소 B(MAO-B)를 억제한다.[47][107][53]필로폰에 의한 시그마 수용체 활성화는 중추신경계 자극제 효과를 촉진하고 뇌 내 신경독성을 촉진시킬 수 있다.[47][53]덱스트로메탐페타민은 더 강한 정신안정제지만 레보메탐페타민말초효과와 반감기가 더 길고 중독자들 사이에서 더 오래 인지되는 효과가 있다.[119][120][121]고량 투여 시 필로폰의 항산화제 모두 유사한 정형외과필로폰 정신착란을 유발할 수 있지만 레보메트암페타민은 정신역학적 효과가 더 짧다.[120][121]

약동학

경구 투여 후 필로폰은 혈류로 잘 흡수되며, 섭취 후 약 3.13~6.3시간 만에 최고 혈장 필로폰 농도가 달성된다.[122]필로폰은 흡입 후와 체내 투여 후 잘 흡수된다.[4]필로폰의 지방질이 높아 다른 자극제보다 혈액뇌장벽을 빠르게 통과해 쉽게 이동할 수 있어 모노아민 산화효소에 의한 분해에 대한 내성이 강하다.[4][122]암페타민 대사물은 10-24시간으로 최고조에 달한다.[4]필로폰은 신장에 의해 배설되며, 소변으로의 배설 속도는 비뇨기 pH의 영향을 많이 받는다.[22][122]구강 복용 시 복용량의 30~54%는 필로폰으로, 1023%는 암페타민으로 소변에서 배설된다.[122]IV 투여 후 약 45%는 필로폰으로, 7%는 암페타민으로 배설된다.[122]필로폰의 반감기는 5~30시간의 범위로 가변적이다.[4]

CYP2D6, 도파민 β-히드록실라아제, 플라빈 함유 모노옥시겐아제3, 부티레이트-CoA 리게아제, 글리신 N-아실전달효소는 필로폰이나 그 대사물을 사람 안에서 대사시키는 것으로 알려진 효소다.[sources 2]주요 대사물은 암페타민과 4-하이드록시메탐페타민이며,[122] 그 외 소대사물로는 4-하이드록시암페타민, 4-하이드록시노레페드린, 4-하이드록시페닐아세톤, 벤조산, 히프산, 노레페드린, 그리고 암페타민의 대사물인 페닐라세톤이 있다.[6][122][123]이들 대사물 중 활성 동조화 물질은 암페타민, 4-히드록시암페타민,[129] 4-히드록시뇨레페드린,[130] 4-히드록시메탐페타민,[122] 노레페드린 등이다.[131]필로폰은 CYP2D6 억제제다.[105]

화학

필로폰은 덱스트로메탐페타민과 레보메탐페타민이라는 두 개의 에나토머를 가진 치랄 화합물이다.상온에서 필로폰의 프리 베이스제라늄 잎의 냄새가 나는 맑고 무색의 액체다.[9]그것은 클로로포름으로 잘못될 만 아니라 디에틸에테르에탄올용해된다.[9]

분해

표백제를 사용한 필로폰의 파괴에 관한 2011년 연구는 효과성이 노출 시간 및 농도와 상관관계가 있다는 것을 보여주었다.[144]토양에 함유된 필로폰이 지속적인 오염물질이라는 연구(2011년)가 1년 동안 나왔다.[145]2013년 폐수 내 생물반응기를 대상으로 한 연구에서 필로폰이 빛에 노출되면 30일 이내에 상당 부분 분해되는 것으로 나타났다.[146]

경혈성 필로폰은 페닐라세톤부터 Leucart[147] 또는 환원성 아미노산 방식으로 제조할 수 있다.[148]Leucart 반응에서 페닐아세톤 1 등가물이 N-메틸포름아미드 2개에 반응하여 필로폰과 이산화탄소, 메틸아민 등의 포밀아미드를 곁들여 생산한다.[148]이 반응에서 이미늄 양이온을 중간으로 형성하여 N-메틸포름아미드(N-methylformamide)의 두 번째 등가물에 의해 감소한다.[148]중간 포밀 아미드는 산성의 수성 조건에서 가수 분해하여 필로폰을 최종 제품으로 산출한다.[148]또는 페닐라세톤은 필로폰을 생산하기 위해 감소 조건 하에서 메틸아민에 반응할 수 있다.[148]

제2차 세계대전 당시 독일군이 사용했던 필로폰 브랜드 페르비틴은 이들 태블릿 용기에 담겨 유통됐다.
2017년 미국의 약물 과다복용 관련 사망자는 7만200명으로, 이 가운데 정신안정제(필로폰 포함) 관련 사망자는 1만333명이었다.[149][150]

필로폰 이전에 발견된 암페타민은 1887년 루마니아 화학자 라즈르 에델레아누에 의해 독일에서 처음으로 합성되었다.[151][152]얼마 지나지 않아 1893년 일본의 화학자 나가이 나가요시에 의해 에페드린에서 필로폰이 합성되었다.[153]그로부터 30년이 지난 1919년, 약리학자 오가타 아키라(小田明太郞)가 붉은 인과 요오드를 이용한 에페드린 감소를 통해 염산필로폰이 합성되었다.[154]

골든트라이앵글(동남아시아) 특히 미얀마 샨주는 미국 및 동아시아와 태평양을 넘나드는 수출용 필로폰 등 야바와 결정성 필로폰으로 생산이 전환되면서 필로폰의 세계 유수의 생산국이다.[164]
