의학적 용법

첼화 요법은 급성 수은,[1][7] 철분( 낫세포 질환탈라세미아의 경우 포함),[8][9] 비소, 납, 우라늄, 플루토늄 및 기타 독성 금속 중독을 포함한 금속 중독에 대한 선호되는 치료법이다. 킬레이트 제제는 제제 및 중독 유형에 따라 정맥주사, 근내주사 또는 구강주사로 투여할 수 있다.[10]

첼팅제

부작용

금속 독성에 의한 위해의 진단에 적절하게 사용될 때, 킬레이트 요법의 부작용은 탈수, 저혈액 칼슘, 신장에 대한 위해, 간 기능 테스트에서 검출될 수 있는 효소 증가, 알레르기 반응, 식이 요법의 낮은 수준 등이다.[16] 부적절하게 투여하면 저칼슘혈증(저칼슘 수치), 신경발달장애, 사망 등의 추가 위험이 있다.[16]

역사

첼화 요법은 I.G.파벤에서 일하는 독일의 화학자 페르디난드 뮌즈에틸렌디아미나메트라아세트산(EDTA)을 처음 합성했던 1930년대 초로 거슬러 올라갈 수 있다.[17] 먼즈는 물 유연제로 구연산 대체품을 찾고 있었다.[17] 첼화요법 자체는 옥스퍼드 대학의 화학자들이 비소에 기초화학무기레즈비언의 해독제를 찾았을 때 2차 세계대전 중에 시작되었다.[17] 화학자들은 EDTA가 납중독 치료에 특히 효과적이라는 것을 알게 되었다.[17]

FDA가 승인한 OTC 킬레이트 제품은 없다."[6]

사회와 문화

1998년 미국 연방거래위원회(FTC)는 미국 의과대학(ACAM)의 웹사이트와 이들이 발행한 브로셔가 허위 또는 근거 없는 주장을 했다고 비난했다. 1998년 12월, 공정위는 킬레이트 요법이 아테롬성 경화증이나 순환기 계통의 다른 질병에 효과적이라는 근거 없는 광고 주장을 하지 못하도록 하는 동의서를 확보했다고 발표했다.[25][27]

2005년 8월, 의사 실수로 인해 킬레이트 치료를 받던 5세 자폐소년이 사망했다.[3] 세 살짜리 비자폐성 소녀와 비자폐성 성인 등 다른 이들은 킬레이트 요법을 받던 중 사망했다.[3] 이러한 사망은 킬레이트 요법 중 저칼슘혈증에 의한 심장마비로 인한 것이었다. 저칼로리혈증은 Na2EDTA(질소 EDTA) 투여에 의해 발생한 것으로 보이는 두 가지 경우, 그리고 세 번째 경우에는 EDTA의 유형을 알 수 없었다.[28][29] 오직 3세 소녀만이 혈중수치가 상승하고 철분 수치와 빈혈이 낮은 것을 발견했는데, 이것은 킬레이트 치료의 일반적인 치료 원인이다.[30] 의전에 따르면,[31] EDTA는 어린이 치료에 사용되어서는 안 된다.[32] 1970년대 이후 IV 관리형 탈부전증(Disodium EDTA)과 관련하여 30명 이상의 사망자는 IV 관리형 탈부전증 EDTA와 관련하여 기록되었다.[3]

대체의학에서 사용

미국 협회는 킬레이트 요법에 대해 "이용 가능한 과학적 증거는 그것이 암과 같은 다른 질환을 치료하는데 효과적이라는 주장을 뒷받침하지 않는다. 킬레이트 요법은 독성이 있을 수 있고 신장 손상, 불규칙한 심장박동, 심지어 사망까지 일으킬 수 있는 잠재력을 가지고 있다."[4]

심혈관질환

1997년 체계적 검토 결과에 따르면 EDTA 킬레이트 치료는 관상동맥질환 치료제로 효과적이지 않으며, 미국 식품의약국(FDA)에 의해 이 사용이 승인되지 않았다.[37]

자폐증
