아세틸살리실산(ASA)으로도 알려진 아스피린통증, 발열, 염증을 줄이기 위해 사용되는 약물이다.[5]아스피린이 치료하는 구체적인 염증 질환으로는 가와사키병, 심막염, 류마티스열 등이 있다.[5]

심장마비 직후에 주어지는 아스피린은 사망 위험을 감소시킨다.[5]아스피린은 또한 더 이상의 심장마비, 빈혈, 고위험군들의 혈액 응고를 예방하는데 도움을 주기 위해 장기간 사용된다.[5]통증이나 발열은 일반적으로 30분 이내에 효과가 나타난다.[5]아스피린은 비스테로이드성 소염제(NSAID)로 다른 NSAID와 유사하게 작용하지만 혈소판의 정상적인 기능도 억제한다.[5]진통제, 항피질, 비스테로이드성 항염증제(NSAID)로 자주 사용되는 아스피린은 혈소판 내 COX 활동을 억제하여 응고 시 혈소판 A2의 생성을 억제하고 혈관 수축과 기관지 수축의 원인이 된다.

한 가지 일반적인 부작용배탈이다.[5]더 큰 부작용으로는 위궤양, 위출혈, 천식 악화 등이 있다.[5]출혈 위험은 나이가 더 많거나, 술을 마시거나, 다른 NSAID를 복용하거나, 다른 혈액 희석제를 복용하는 사람들 사이에서 더 크다.[5]아스피린은 임신 말기에는 권장하지 않는다.[5]레이 증후군의 위험성 때문에 일반적으로 감염 어린이에게는 권장되지 않는다.[5]많은 복용량은 귀에 울림을 유발할 수 있다.[5]

화학적 특성

아스피린은 아세트산 암모늄이나 아세트산, 탄산염, 구연산염 또는 알칼리 금속수산화물 용액에서 빠르게 분해된다.건조한 공기에서는 안정적이지만, 점차 아세트산, 살리실산에 습기와 접촉하면서 가수분해된다.알칼리 용액에서는 가수분해가 빠르게 진행되며 형성된 명확한 용액은 전적으로 아세테이트와 살리실산염으로 구성될 수 있다.[16]

분말공기 혼합물은 폭발적일 수 있기 때문에, 아스피린 알약을 생산하는 공장들은 건물 안에서 공기 중에 떠다니는 분말의 양을 조절해야 한다.미국 국립산업안전보건연구소(NIOSH)는 미국에서 권장 노출 한도를 5mg/m3(시간 가중 평균)로 설정했다.[17]1989년, 산업안전보건청(OSHA)은 아스피린에 대한 법적 허용 노출 한도를 5mg/m로3 설정했지만, 이는 1993년 AFL-CIOOSHA 결정으로 무효화되었다.[18]

합성

살리실산의 아세틸 유도물질인 아스피린은 희고 결정적이며 약한 산성 물질로 용해점은 136 °C(277 °F),[4] 비등점은 140 °C(284 °F)이다.[22]그것의 산성 분해 상수(pKa)는 25°C(77°F)에서 3.5이다.[23]

다형성

작용기전

메커니즘 발견

1971년 당시 런던의 왕립 외과대학에 고용된 영국의 약리학자 로버트 베인은 아스피린이 프로스타글란딘트롬박산기의 생산을 억제하는 것을 보여주었다.[26][27]이 발견으로 그는 수네 베르그스트룀벵트 잉게마르 사무엘손과 공동으로 1982년 노벨 생리의학상을 받았다.[28]

프로스타글란딘과 트롬복소네

아스피린이 프로스타글란딘과 트롬박스탄의 생산을 억제하는 능력은 프로스타글란딘과 트롬박스탄 합성에 필요한 사이클록시제네아제(COX, 공식적으로는 프로스타글란딘-엔도페록세아제, PTGS) 효소의 되돌릴 수 없는 불활성화 때문이다.아스피린은 아세틸 그룹이 PTGS 효소의 활성 부위의 세린 잔류물에 공칭적으로 부착되는 아세틸화 물질(Suffered inhibition) 역할을 한다.이것은 아스피린을 가역성 억제제인 다른 NSAID(diclofenac, ibuprofen 등)와 다르게 만든다.

저선량 아스피린 사용은 불가역적으로 혈소판 A2 형성을 차단하여 해당 혈소판의 수명(8~9일) 동안 혈소판 통합에 억제 효과를 발생시킨다.이 항혈전 특성은 아스피린을 심장마비, 불안정한 협심증, 허혈성 뇌졸중 또는 일시적인 허혈성 발작을 일으킨 사람들의 심장마비 발생률을 줄이는 데 유용하게 만든다.[29]하루 40mg의 아스피린은 프로스타글란딘 I2 합성에 거의 영향을 받지 않고 급성 발작인 최대 트롬박산 A의2 방출을 억제할 수 있다. 그러나 추가적인 억제를 얻기 위해서는 더 많은 양의 아스피린이 필요하다.[30]

체내에서 생성되는 국소호르몬인 프로스타글란딘은 뇌로 통증정보 전달, 저체온도조절기 변조, 염증 등 다양한 효과가 있다.혈소판은 혈전을 형성하는 혈소판의 집적을 책임진다.심장마비는 주로 혈액 응고에 의해 발생하며, 적은 양의 아스피린은 2차 급성 심근경색을 예방하기 위한 효과적인 의학적 개입으로 보여진다.[31]

COX-1 및 COX-2 억제

적어도 두 가지 다른 유형의 사이클록시제인 COX-1과 COX-2는 아스피린에 의해 작용한다.아스피린은 COX-1을 불가역적으로 억제하고 COX-2의 효소 활성을 수정한다. COX-2는 보통 프로스타노이드(prostanoid)를 생산하는데, 대부분은 염증이다.아스피린 변형 PTGS2(프로스타글란딘-엔도페록시드 신타아제2)는 리판신을 생성하는데, 대부분이 항염증이다.[32][verification needed]신형 NSAID 약품인 COX-2 억제제(콕시브)는 위장 부작용 발생을 줄이기 위해 PTGS2만 억제하도록 개발되었다.[9]

PTGS2 억제제가 심장마비와 뇌졸중의 위험을 증가시킨다는 증거가 나온 후 로페콕시브(Vioxib)와 같은 여러 COX-2 억제제가 시장에서 철수되었다.[33][34]체내 미세조직에 라이닝되는 내피세포는 PTGS2를 표현하기 위해 제안되며, 선택적으로 PTGS2를 억제함으로써 혈소판의 PTGS1이 영향을 받지 않기 때문에 트롬박산 수준과 관련하여 프로스타글란딘 생산(특히 PGI2; 프로스타시클린)을 하향 조절한다.따라서 PGI2의 보호항응력이 제거되어 혈전 및 관련 심장마비 및 기타 순환기 질환의 위험이 증가한다.혈소판은 DNA가 없기 때문에 아스피린이 효소를 불가역적으로 억제하면 새로운 PTGS를 합성할 수 없게 되는데, 이는 가역 억제제와의 중요한 차이점이다.

더욱이 아스피린은 프로스타글란딘과 같은 친염증 제품을 형성하는 COX-2의 능력을 억제하면서 이 효소의 활동을 프로스타글란딘 형성의 사이클록시제나아제(cycloxygenase)에서 리폭시제나제(lipoxy genase)와 같은 효소로 전환시킨다:아스피린 처리된 COX-2는 다양한 다불포화 지방산을 수산화산을 수산화물로 대사한다.아스피린-아스피린-리판신, 아스피린-아스피린-아스피린-리판신, 아스피린-아스피린-아스피린-아스피린-매레신 같은 전문 프로레졸링 매개체에 대사된다.이 중재자들은 강력한 항염증 활동을 가지고 있다.이러한 아스피린이 유발한 COX-2의 사이클록시제네아제로부터 리폭시제네아제 활성으로의 전환과 그에 따른 전문 프로레졸링 매개체의 형성이 아스피린의 항염증 효과에 기여하는 것이 제안된다.[35][36][37]

추가 메커니즘

약동학

아세틸살리실산은 약한 산으로, 경구 투여 후 위장에서 이온화되는 경우는 거의 없다.아세틸살리실산은 위장의 산성 조건에서 세포막을 통해 빠르게 흡수된다.pH가 증가하고 소장의 표면적이 넓어지면 아스피린이 더 많이 이온화되기 때문에 아스피린이 그곳에서 더 느리게 흡수된다.콘크리트의 형성으로 인해 아스피린은 과다복용 시 훨씬 더 천천히 흡수되며, 플라즈마 농도는 섭취 후 최대 24시간까지 상승할 수 있다.[48][49][50]

혈중 살리실산의 약 50-80%는 인간 혈청 알부민에 결합되어 있고, 나머지는 활성 이온화 상태에 머물러 있으며, 단백질 결합은 농도에 의존한다.결합 부위의 포화상태는 더 많은 자유 살리실산과 독성 증가로 이어진다.분배량은 0.1–0.2 L/kg이다.산도증은 살리실산염의 조직 침투가 강화되어 분포의 부피를 증가시킨다.[50]

살리실산 치료용량의 80%가 에서 대사된다.글리신과의 결합살리실루르산을 형성하고, 글루쿠론산과 함께 두 개의 다른 글루쿠로니드 에스테르를 형성한다.아세틸 그룹이 그대로 있는 결합을 아킬 글루쿠로니드라고 하며, 탈 아세틸화 결합은 페놀 글루쿠로니드라고 한다.이러한 신진대사 경로는 한정된 용량밖에 없다.소량의 살리실산도 젠티스산에 히드록시화된다.많은 살리실산염 용량으로, 신진대사 경로가 포화 상태가 되고 신장 배설의 중요성이 점점 높아짐에 따라, 운동학은 1차에서 0차순으로 전환된다.[50]

살리실산은 주로 신장에 의해 살리실루르산(75%), 유리 살리실산(10%), 살리실릭페놀(10%), 아킬 글루쿠로니데스(5%), 젠티산(<1%), 디히드록시벤조산(23-dihydroxybenzoic acid)으로 배설된다.[51]소량 선량(성인에서 250mg 미만)을 섭취하면 모든 경로가 1차 운동학에 의해 진행되며, 제거 반감기는 약 2.0시간에서 4.5시간이다.[52][53]살리실산(salicylate)을 더 많이 섭취할 경우(4g 이상) 살리실루르산 및 살리실 페놀 글루쿠로니드의 형성과 관련된 생체역변성 경로가 포화 상태가 되기 때문에 반감기가 훨씬 길어진다(15~30시간).[54][55]살리실산의 신장 배설은 신진대사 경로가 포화되면서 점점 중요해지고 있는데, 이는 비뇨기 pH의 변화에 극도로 민감하기 때문이다.소변 pH를 5에서 8로 늘렸을 때 신장 간극이 10~20배 증가한다.비뇨기 알칼리화의 사용은 살리실산 제거의 이 특별한 측면을 이용한다.[56]치료용량에 아스피린을 단기 복용할 경우 글루머론증이나 간경변으로 환자가 병이 났을 때 가역성 급성 신장 손상을 유발할 수 있는 것으로 나타났다.[57]만성 신장 질환을 앓고 있는 일부 환자와 울혈성 심부전을 앓고 있는 일부 어린이들을 위한 아스피린이 억제되었다.[57]

아스피린은 류마티스성 관절염, 심막염, 가와사키병 등 발열, 통증, 류마티스성 열, 염증성 질환 등 여러 질환의 치료에 쓰인다.[69]아스피린을 적게 복용하면 심장마비로 인한 사망 위험이나 심혈관 질환이 있는 사람에게 뇌졸중 위험이 감소하지만, 그렇지 않으면 건강한 노인에게는 그렇지 않은 것으로 나타났다.[70][71][72][73][74]아스피린이 대장암 예방에 효과적이라는 증거가 있지만, 이 효과의 메커니즘은 불분명하다.[75]미국에서는 심혈관 질환의 위험이 10% 이상인 50~70세에게 저선량 아스피린은 출혈 위험이 증가하지 않고, 그렇지 않으면 건강한 것으로 간주된다.[76]

통증

아스피린은 일반적으로 이부프로펜보다 열등하다고 여겨지지만 급성 통증에 효과적인 진통제로, 위장관 출혈을 일으킬 가능성이 높기 때문이다.[77]아스피린은 일반적으로 근육 경련, 팽창, 위팽창 또는 급성 피부 자극으로 인한 통증에 효과적이지 않다.[78]다른 NSAIDs와 마찬가지로, 아스피린과 카페인조합은 아스피린만 사용하는 것보다 약간 더 큰 통증 완화를 제공한다.[79]아스피린의 발육이 아스피린보다 빨리 통증을 완화시켜 편두통 치료에 유용하게 쓰인다.[80][81]국소 아스피린은 신경병통 치료에 효과적일 수 있다.[82]

아스피린은 그 자체로 또는 복합제제로서, 특정 종류의 두통을 효과적으로 치료하지만, 그 효능은 다른 사람들에게는 의심스러울 수 있다.2차적 두통은 다른 장애나 트라우마로 인한 두통을 의미하며, 의료 사업자에 의해 즉시 치료되어야 한다.일차적 두통 중 국제두통분류법긴장성 두통(가장 흔함)과 편두통, 군발성 두통을 구분한다.아스피린이나 다른 처방전 없이 살 수 있는 진통제는 긴장성 두통 치료에 효과가 있는 것으로 널리 알려져 있다.[83]아스피린은 특히 아스피린/파라세타몰/카페인 조합의 성분으로서 편두통 치료에 있어 일선요법으로 여겨지며, 수마티프탄의 저선량과 견줄 만하다.편두통이 처음 시작될 때 멈추는 데 가장 효과적이다.[84]

통증을 조절하는 능력처럼 아스피린이 을 조절하는 능력은 COX의 되돌릴 수 없는 억제를 통해 프로스타글란딘 시스템에 작용하기 때문이다.[85]성인에서 아스피린이 항정신병약으로 쓰이는 것은 잘 확립되어 있지만, 미국가족의사협회, 미국소아과학원, 식품의약품안전청 등 많은 의료사회와 규제기관은 레이의 발열 위험이 있으므로 아동의 발열 치료에 아스피린을 사용하는 것을 강력히 권고하고 있다. 바이러스나 박테리아 감염의 에피소드 동안 어린이들에게 아스피린이나 다른 살리실레이트의 사용과 관련된 드물지만 종종 치명적인 질병인 신드롬.[86][87][88]1986년 미국 식품의약국(FDA)은 어린이들에게 레이 증후군의 위험 때문에 아스피린이 함유된 모든 약물에 어린이와 청소년들에게 사용을 금지하는 라벨 표시를 의무화했다.[89]

염증

아스피린은 류마티스 관절염과 같은 염증성 질환의 치료뿐만 아니라 급성 [90]염증과 장기 염증 모두에 대한 항염증제로 사용된다.[69]

심장마비 및 뇌졸중

아스피린은 심장마비를 겪은 사람들의 치료에 있어 중요한 부분이다.[91]일반적으로 70세 이상 노인 등 다른 건강상의 문제가 없는 사람이 일상적으로 사용하는 것은 권장하지 않는다.[92]

이미 심장마비나 뇌졸중을 앓은 사람의 경우 아스피린을 매일 2년 동안 복용하면 심혈관 질환(심장마비, 뇌졸중, 사망)은 50명 중 1명이 예방할 수 있었지만 400명 중 1명에게서 비치과성 출혈 문제가 발생하기도 했다.[93][94][31]1차 예방의 초기 아스피린 실험에서 얻은 데이터는 저선량 아스피린이 사람들에게 더 유익하다는 것을 시사했다. 70 kg, 고선량 아스피린은 70 kg에 더 유익하다.[95]그러나 최근 더 많은 실험에서 저선량 아스피린은 체중이 적은 사람들에게 더 효과적이지 않으며 체중이 많은 사람들에게 더 높은 선량의 아스피린의 효과를 결정하기 위한 더 많은 증거가 요구되고 있다.[96][97][98]미국 예방 서비스 태스크 포스(USPSTF), 2016[업데이트]에, 심장 혈관 질환과 대장 암의 1차 예방 위한 어른들을 위해에서 소량 아스피린 사용 시작 10%이상 10년 심혈관 질환(CVD) 위험이 50에서 59년, 출혈 위험 증가가 살아온 삶에 expectanc 있지 않다 추천했다.의 y최소 10년 이상, 매일 최소 10년 동안 저칼로리 아스피린을 복용할 용의가 있다.[99][76]다만 2021년 USPSTF는 부작용 위험이 잠재적 편익보다 크다는 점을 들어 40~50대 성인의 일차예방을 위해 일상적으로 아스피린을 사용하지 말라고 권고했다.[100]60세 미만의 개인들은 매일 아스피린을 복용하기 전에 먼저 의료 제공자와 상담해야 한다.

심장질환 전과가 없는 사람에게 아스피린은 비임상 심근경색 위험을 감소시키지만 출혈 위험은 증가시키고 전반적인 사망 위험은 변화시키지 않는다.[101][102][103]특히 5년에 걸쳐 심혈관 질환의 위험을 265년에 1명 감소시켰고 210년에 1명 증가시켰다.[103]

예를 들어, 아스피린은 심장마비나 뇌졸중의 위험이 낮은 사람들에게 거의 도움이 되지 않는 것 같다. 예를 들어, 이러한 사건들의 이력이 없거나 질병이 있는 사람들이다.[104]일부 연구에서는 사례별로 아스피린을 권장하는 반면,[104][99] 다른 연구에서는 위장 출혈과 같은 다른 사건의 위험이 잠재적 편익을 능가할 만큼 충분하다고 제안하고 아스피린을 전적으로 1차 예방에 사용하지 말라고 권고했다.[105]아스피린은 심혈관 질환 예방을 위한 폴리필 성분으로도 제시됐다.[106][107]

아스피린을 예방에 사용하는 것을 복잡하게 만드는 것은 아스피린 저항성의 현상이다.[108][109]내성이 있는 사람에게는 아스피린의 효능이 떨어진다.[110]몇몇 저자들은 아스피린에 내성이 있는 사람들을 식별하기 위한 방법을 시험하는 방법을 제안했다.[111]

관상동맥 스텐트 배치 등 경피적 관상동맥 중재(PCI) 이후 미국 보건의료연구품질국(A Agency for Healthcare Research and Quality) 가이드라인은 아스피린을 무기한 복용할 것을 권고하고 있다.[112]흔히 아스피린은 클로피도그렐, 프라수그렐, 티카그렐러ADP 수용체 억제제와 결합해 혈액 응고를 예방한다.이를 이중 항혈소판치료(DAPT)라고 한다.DAPT의 기간은 CURE와 PENDIGY 연구 후에 미국과 유럽연합 지침에서 권고되었다.2020년에 [115]Khan 등의 체계적 검토와 네트워크 메타분석 결과, 단기(< 6개월) DAPT에 이어 선별된 환자에서도 P2Y12 억제제, 고위험 환자에서도 장기(> 12개월) DAPT의 수혜가 유망했다.결론적으로 PCI 이후 DAPT의 최적 지속시간은 여러 환자 관련 요인 및 절차 관련 요인을 고려하여 허혈성 사건의 위험과 출혈 사건의 위험을 초과한 후 개인화해야 한다.게다가 아스피린은 DAPT가 끝난 후에 무기한으로 계속되어야 한다.[116][117][118]

암예방

아스피린은 암에 걸릴 위험과 암으로 죽을 위험 모두를 줄일 수 있다.[119]대장암(CRC)의 위험을 낮출 수 있는 실질적인 증거가 있지만,[75][120][121][122] 이러한 혜택을 보려면 적어도 10~20년 동안 복용해야 한다.[123]그것은 또한 자궁내막암,[124] 유방암, 전립선암의 위험을 약간 줄일 수 있다.[125]

2019년까지 메타분석을 한 결과 아스피린을 복용하는 것과 대장암, 식도암, 위암 발병률이 낮은 것 사이에 연관성이 있다고 밝혔다.[129]

2021년 미국 예방 서비스 태스크포스는 암 예방에 아스피린을 사용하는 것에 대해 문제를 제기했다.위약군보다 아스피린 치료군에서 암과 관련된 사망 위험이 더 높았던 2018 ASPREE(어르신 사건 감소에서의 아스피린) 임상시험 결과를 주목한다.[130]

정신의학

조울증

아스피린은 항염증 성질을 가진 여러 다른 제제와 함께 중증 정신질환의 병원체에서 염증이 발생할 수 있는 가능성에 비추어 조울증 환자들의 우울증 증상에 대한 추가 치료제로 용도 변경되었다.[131]그러나 메타분석적 증거는 극소수의 연구에 기초하고 있으며 양극성 우울증 치료에 아스피린의 효능을 전혀 제시하지 않는다.[131]따라서 생물학적 근거에도 불구하고 양극성 우울증 치료에 있어 아스피린과 항염증제의 임상적 관점은 여전히 불확실하다.[131]

치매

아스피린이 치매를 예방한다는 증거는 없다.[132]

기타 용도

아스피린은 급성 류머티즘열의 발열과 관절염 증상에 대한 1차 치료법이다.치료는 종종 1~2주 정도 지속되며, 더 긴 기간 동안 거의 나타나지 않는다.발열과 통증이 가라앉은 후 아스피린은 심장 합병증과 잔류 류머티즘 심장질환의 발생을 줄이지 않기 때문에 더 이상 필요하지 않다.[133][134]그동안 나프록센은 아스피린 못지않은 효능과 독성이 있는 것으로 나타났지만 임상경험이 한정돼 있어 2선 치료제로만 나프록센을 추천한다.[133][135]

류마티스열과 함께, 가와사키 병은 효능에 대한 높은 품질의 증거가 부족함에도 불구하고[136] 어린이들에게 아스피린 사용에 대한 몇 안 되는 징후 중 하나로 남아 있다.[137]

저선량 아스피린 보충제는 에클램피아 전 예방에 사용했을 때 적당한 혜택을 준다.[138][139]이 혜택은 임신 초기에 시작했을 때 더 크다.[140]

저항

어떤 사람들에게는 아스피린이 다른 사람들처럼 혈소판에 강한 영향을 미치지 않는데, 이것은 아스피린 저항성 또는 불감증으로 알려져 있다.한 연구는 여성이 남성보다 저항성이 더 높다는 것을 제시했고,[141] 다른 2,930명의 조사 결과 28%가 저항성이 있는 것으로 밝혀졌다.[142]100명의 이탈리아인을 대상으로 한 연구에서 명백한 31%의 아스피린 저항성 피험자 중 5%만이 진정 저항성이 있으며, 다른 사람들은 준수하지 않는 것으로 나타났다.[143]400명의 건강한 자원 봉사자들을 대상으로 한 또 다른 연구에서는 진정 저항력이 있는 피험자는 발견되지 않았지만, 일부는 "지연되고 감소된 약물 흡수를 반영하여 허무맹랑한 저항력을 가지고 있었다"고 밝혔다.[144]

메타 분석과 체계적 검토는 실험실에서 확인된 아스피린 저항성이 심혈관 질환과 신경혈관 질환에서 더 나쁜 결과의 비율을 증가시킨다는 결론을 내렸다.[145][142][146][147][148][149]심혈관계와 신경혈관을 둘러싼 연구가 대부분이지만 정맥혈전증 예방을 위해 아스피린을 사용하는 정형외과 수술 후 아스피린 내성 위험에 대한 연구가 활발해지고 있다.[150]특히 전체 고관절과 무릎 관절염 후 정형외과 수술에서 아스피린 저항성은 아스피린 저항성의 위험인자가 정맥 혈전증과 골관절염의 위험인자가 되기 때문에 흥미롭다. 즉, 전체 고관절 또는 무릎 관절염의 연속이다.이러한 위험 요소들 중 일부는 비만, 고령화, 당뇨병, 이상지질혈증, 염증성 질환을 포함한다.[150]다만 심혈관질환이나 신경혈관질환과 달리 정형외과 수술에서 아스피린 저항성의 발병률에 대한 확인도 없고 임상적 의미에 대한 확인도 없다.

수의학

아스피린은 항응고제로 수의학에서 쓰이거나 근골격계 염증이나 골관절염과 관련된 통증을 완화하기 위해 쓰이기도 한다.아스피린은 위장병을 포함한 부작용이 일반적이기 때문에 수의사의 직접 감독 하에 동물들에게만 주어야 한다.어떤 종에서든 아스피린 과다 복용으로 인해 살리실산염 중독이 발생할 수 있는데, 이 중독은 출혈, 발작, 혼수상태, 심지어 죽음으로까지 특징지어진다.[151]

개는 고양이보다 아스피린을 더 잘 참을 수 있다.[152]고양이는 아스피린 분비를 돕는 글루쿠로니드 접합제가 부족하기 때문에 천천히 아스피린을 대사하는데, 투여 간격을 제대로 두지 않으면 독성이 생길 수 있다.[151][153]고양이에게 4주 동안 48시간마다 25mg/kg의 아스피린을 투여했을 때 독성의 임상 징후는 발생하지 않았지만,[152] 통증 및 발열 완화 및 고양이 혈액 응고 질환 치료 권장량은 48시간마다 10mg/kg으로 대사작용이 가능하다.[151][154]

복용량

성인용 아스피린 알약은 국가별로 조금씩 다른 표준 크기로 생산되는데, 예를 들어 영국에서는 300mg, 미국에서는 325mg(또는 5곡)이 생산된다.더 작은 선량은 이러한 표준에 기초한다. 예를 들어 75mg과 81mg 알약.81와그램(1+14-3) 알약은 원래 유아와 어린이에게 투여하기 위한 것이 아니었기 때문에 일반적으로 "아기 아스피린" 또는 "아기 힘"이라고 불린다.[155]75mg과 81mg 알약 사이의 약간의 용량 차이로 인해 의학적 의미는 발생하지 않는다.복리후생에 필요한 선량은 사람의 체중에 따라 달라 보인다.[95]몸무게가 70kg(154lb) 미만인 환자의 경우 저선량이 심혈관 질환 예방에 효과적이며, 이 중량 이상 환자의 경우 고선량이 필요하다.[95]

일반적으로 성인의 경우 발열이나 관절염으로 하루 4회 복용하는데,[156] 류마티스열 치료에 역사적으로 사용된 최대 일일 복용량에 가까운 복용량을 복용한다.[157]관상동맥질환이 있거나 의심되는 사람의 심근경색(MI) 예방을 위해 매일 1회 훨씬 더 낮은 선량을 복용한다.[156]

가와사키 병에 걸린 어린이의 경우 아스피린은 체중을 기준으로 복용하는데, 처음에는 하루에 4번 최대 2주 동안 복용한 후, 매일 6~8주 동안 더 낮은 복용량으로 복용한다.[160]

역효과

미국 식품의약국(FDA)은 2020년 10월, 양수가 낮은 태아의 신장 질환 위험을 설명하기 위해 모든 비스테로이드성 항염증 약물에 대해 약물 라벨을 업데이트하도록 요구했다.[161][162]그들은 임신 20주 혹은 그 이후에 임산부의 NSAIDs를 피하라고 권고한다.[161][162]권고안 중 한 가지 예외는 저선량 81mg 아스피린을 임신 중 어느 시점에나 건강관리 전문가의 지시에 따라 사용하는 것이다.[162]

콘트라인커뮤니케이션

위장

장내 코팅 외에도, "버퍼링"은 회사들이 위장 출혈의 문제를 완화하기 위해 노력했던 또 다른 주요 방법이다.완충제는 완충 아스피린의 효능에 대해서는 논란이 있지만, 위벽에 아스피린이 집중되지 않도록 하여 효과가 있다.제산제에 사용되는 거의 모든 완충제가 사용될 수 있다; 예를 들어, Bufferin은 산화마그네슘을 사용한다.다른 준비물들은 탄산칼슘을 사용한다.[177]알카-셀처(Alka-Seltzer)에서 발견되는 중탄산나트륨(bicarbonate)을 예로 들 수 있다.[178]

비타민 C와 함께 복용하는 것은 위벽을 보호하는 방법으로 조사되어 왔다.비타민 C와 아스피린을 같은 용량으로 복용하면 아스피린만 복용하는 것에 비해 발생하는 위 손상의 양을 줄일 수 있다.[179][180]

망막정맥폐쇄

중심 망막정맥폐쇄(CRVO), 가지 망막정맥폐쇄(BRVO) 등 망막정맥폐쇄(RVO) 환자에 대한 추가 약물로서 아스피린을 처방하는 것이 안과 전문의(오판학)들 사이에 널리 퍼져 있는 습관이다.이러한 널리 사용되는 이유는 주요 전신 정맥 혈전 장애에서 효과가 입증되었다는 증거로서, 다양한 종류의 망막 정맥 폐색에도 유사하게 유익할 수 있다고 가정되어 왔다.

그러나 700명에 가까운 환자의 데이터를 바탕으로 한 대규모 조사에서는 "아스피린이나 다른 항혈소판 집적제나 항응고제가 보호나 유익한 효과의 어떠한 증거도 없이 CRVO와 헤미-CRVO 환자들에게서 시각적 결과에 악영향을 끼친다"[181]는 것을 보여주었다.왕립 안과대학교를 포함한 몇몇 전문가 단체들은 RVO 환자들을 위한 항혈전제(아스피린) 사용을 반대한다고 권고했다.[182]

중심 효과

아스피린의 대사물인 살리실산을 다량 복용하면 아라키돈산NMDA 수용체에 대한 작용을 통해 랫드 실험에 기초한 일시적인 이명(귀의 고리)이 발생한다.[183]

레이즈 증후군

레이증후군은 드물지만 급성뇌병증지방간으로 특징지어지는 중증질환으로 소아나 청소년이 발열이나 다른 질병이나 감염으로 아스피린을 투여받을 때 발생할 수 있다.1981년부터 1997년까지 미국 질병관리본부에 보고된 18세 미만 인구에서 1207건의 레이 증후군이 있었다.이 중 93%가 레이 증후군이 발병하기 전 3주 동안 아프다고 신고했는데, 가장 흔한 것은 호흡기 감염, 수두, 설사 등이다.살리실레이트는 검사 결과가 보고된 어린이의 81.9%에서 검출이 가능했다.[184]레이 증후군과 아스피린과의 연관성이 보고되고 이를 예방하기 위한 안전조치(의사장 경고, 아스피린 함유 의약품의 라벨 변경 포함)가 시행된 후, 미국에서 어린이들이 복용한 아스피린은 레이 증후군의 보고 건수와 마찬가지로 상당히 감소했다.소아 아스피린 사용에 대한 경고가 발표된 후 영국에서도 유사한 감소세가 발견되었다.[184]미국 식품의약국(FDA)은 현재 열이 있는 12세 이하 어린이에게는 아스피린(혹은 아스피린 함유 제품)을 주지 말 것을 권고하고 있으며,[170] 영국 국립보건원은 의사의 조언이 아니면 아스피린을 복용하지 말 것을 권고하고 있다.[185]

스킨

기타 부작용

하루 평균 아스피린 투약량이 270mg인 집단을 대상으로 한 연구에서는 1만명당 12건의 뇌내출혈(ICH)의 평균 절대위험 증가를 추정했다.[199]이에 비해 심근경색 절대위험감소 추정치는 1만명당 137건, 허혈성 뇌졸중 1만명당 39건의 감소가 있었다.[199]이미 ICH가 발생한 경우, 아스피린 사용은 사망률이 더 높아지는데, 하루 약 250mg의 복용량이 발생하여 ICH 약 2.5년(95% 신뢰구간 1.3~4.6) 이후 3개월 이내에 상대적 사망위험이 발생한다.[200]

아스피린과 다른 NSAIDs는 프로스타글란딘 합성의 억제를 통해 저선염성 피하팔도스테론 상태를 유도함으로써 비정상적으로 높은 칼륨 수치를 유발할 수 있지만, 이러한 작용제들은 보통 정상적인 신장기능과 유혈상태의 설정에서 자체적으로 고칼륨혈증을 유발하지는 않는다.[201]

과다 복용

상호작용
